精品论文 参考文献 PPH术后继发直肠肛门肌瘤的防治 吴正勇（湖北武汉市第八医院 430000） 【中图分类号】R735.3【文献标识码】A【文章编号】1672-5085（2012）24-0187-02 【摘要】目的 探讨PPH术后继发直肠肛门肌瘤的发病机制和相应的防治措施，减少并发症的发生率，提高广大肛肠科医师的诊治水平，更好的运用此项新技术。方法 通过收集诊治病人的资料，了解病人的病情，采用再手术的方法去除肛门肌瘤。结果 根据病人临床表现究其原因表现为排异反应、疤痕体质、手术操作不熟练不必要的缝合、吻合口血肿机化、吻合口感染等因素造成，提出防治措施注意手术操作方法、养成良好排便习惯便后坐浴，肛门伤口换药及早取出未完全脱落钛钉，提早进行扩肛治疗。结论 PPH术作为治疗痔病的常规方法之一，仍存在一些并发症，还需进行技术改进，推广运用。 【关键词】痔 吻合器痔上黏膜环切术 肛门肌瘤 并发症 预防 治疗 【Abstract】　Discuss PPH ski11 and carry on and take p1ace the morbidity mechanism of skin tumours of rectum rectum and correSPOnding prevention measure，reduce the incidence of comp1ication ，improve the competence of making a diagnosis and giving treatment of the enteric departments d℃tor of vast anus ，this new techno1ogy of better app1ication.Method ：Through co11ecting the materia1s which treat the patient ，find out about the patients condition ，remove rectum skin tumour by performing the operation again.Resu1t ：according to patients c1inica1 manifestation ，trace it to its cause and is shown as and arranged the different reSPOnse，scar physique.The operation is operated raw1y ，unnecessary stitches，identica1 hematoma machines ，factor ，put forward the prevention measure ，pay attention to the operation working technique ，form the g00d habit ofdefecating ，the bath defecates，sits ，the rectum wound changes dressings ，take out the titanium not tota11y coming off to nai1 ear1y ，expand the anus treatment ahead of time Conc1usion ：PPH surgery regarded as one of the conventiona1 methods which treats pi1e ，some comp1ications sti11 exist ，Ana1 1eiomyoma，need to carry on techno1ogy to improve，popu1arize and use. pph自问世以来,以其微创、无痛和有效的特点,在国内很快得到了推广。但PPH作为一种手术,仍然是一种创伤性的治疗手段,术后会出现一些并发症,因此并发症的预防和处理是临床上需要总结和研究解决的课题之一[1]。一项多中心临床研究结果显示，PPH术后并发症高达9.5%[2],而继发直肠肛门肌瘤并发症为8%-11.3%。一般肛门肌瘤形成约3个月至一年半时间，半年内形成约占85%；肛门肌瘤少发1-2个肌瘤，多发6-7个肌瘤，肛门肌瘤形态多呈圆形或椭圆形，质地大多较非PPH形成肛门肌瘤硬，大部分不带蒂，并肌瘤反复刺激后易增大，引起直肠狭窄给患者带来了很大痛苦，排便脱出，严重者致患者无法排，不能进食。为了防止并发症的发生，更好的推广应用新技术，现回顾我科2006年3月至2008年11月PPH术后132例继发
