精品论文 参考文献 探讨奥瑞姆自护模式在高龄髋关节置换术后患者中的应用 谢芳英（四川省医学科学院四川省人民医院骨科 四川成都 610072） 【摘要】目的 探讨奥瑞姆自护模式在高龄髋关节置换患者术后护理中的应用效果。方法 将90例行人工髋关节置换的高龄患者按2：1比例随机分为观察组和对照组。对照组按照常规进行护理，观察组在常规护理基础上运用奥瑞姆自护理论的三个护理系统护理,尽量让患者参与到力所能及的自理活动中。结果 观察组术后早期髋关节活动度、髋关节功能评分均高于对照组(P＜0.05)，满意率调查评估有明显提高。结论 在髋关节置换术后应用奥瑞姆自护模式促进了患者功能恢复, 减少住院天数，提高了护理质量及满意度。 【关键词】奥瑞姆自护模式 人工关节置换 术后护理 股骨粗隆间骨折 【中图分类号】R395.5【文献标识码】A【文章编号】1672-5085（2012）19-0075-02 Discussion the using of Orems self-care mode in hip replacement patients Xie Fangying（Department of orthopedics，Sichuan Academy of medical sciences amp; Sichuan Provincial Peoples Hospital，Chengdu 610072，China） 【Abstract】Objective To investigate the effect of Orems self-care mode in elderly hip replacement patients. Methods：90 cases of hip replacement patients, according to the 2 : 1 scale were randomly divided into self-care group and control group. The experimental group were given conventional care, the self-care group were used the three care system of Orems self-care mode additional, try to enable patients to participate in health decision-making and means of self-care activities. Results：The patients early postoperative hip joint range of motion , hip function scores in experimental group were better than that control group(P＜0.05), and satisfaction survey is improved obviously. Conclusion：Apply the Orems self-care mode in elderly hip replacement patients could promote the functional recovery, shorten the length of stay, improve nursing quality and satisfaction. 【Key words】Orems self-care mode Artificial joint replacement Postoperative care Intertrochanteric fractures 股骨粗隆间骨折是骨科临床上较常见骨折之一，发病率约占髋部骨折的31%～51%[1]，如治疗不当，患者可能留有肢体残疾、生活不能自理，严重影响患者生存质量，其病死率可高达15%～20%[2]。目前随着人们生活水平及医疗条件、就医意识的提高，手术成为了治疗此类骨折的主要手段，术后的专科护理也就显得尤为重要。美国著名护理理论家奥瑞姆的自理理论强调护理的最终目的是恢复和增强人的自理能力[3]，对护理实践有着重要的指导作用，为术后护理提供了理论依据。我科自2009年03月～2011年02月应用奥瑞姆自护模式对高龄粗隆间骨折行髋关节置换的患者进行护理,效果满意,现报道如下。 1资料与方法 1.1 一般资料 本组高龄粗隆间骨折行髋关节置换的患者90例(94髋)，男39例，女51例，年龄76～94岁，平均83岁。受伤原因：行走时不慎跌倒84例
