精品论文 参考文献 腹腔镜直肠癌根治术在不同年龄患者中受益程度的对比分析 温雨琼 张倩（四川省宜宾市第一人民医院 644000） 【中图分类号】R735.3+7【文献标识码】A【文章编号】1672-5085（2012）20-0097-02 【摘要】目的 探讨腹腔镜直肠癌根治术在不同年龄患者中受益程度的差别。方法 我院外科治疗的直肠癌患者128例，其中老年患者与中青年患者各64例，将老年患者与中青年患者均平均分为腹腔镜组和开腹组各32例。对两组患者给予综合性的围术期护理干预。比较两组患者的手术时间、术中出血量、术后排气时间、住院时间、使用镇痛药比例、出院时生活自理比例以及并发症。结果 老年腹腔镜组患者除手术时间外，各项观察指标均明显优于老年开腹组患者。中青年腹腔镜组患者术中出血量、术后排气时间、住院时间、使用镇痛药比例均明显优于中青年开腹组患者，但在出院时生活自理比例以及并发症方面，二者无明显差异。结论 老年患者以及中青年患者应用腹腔镜直肠癌根治术均具有疗效确切、安全可靠的特点，通过比较，老年患者在腹腔镜直肠癌根治术中受益更多。 【关键词】 腹腔镜直肠癌根治术 不同年龄患者 受益程度 Comparative Analysis Between Laparoscopic and Open Radical Operation in Patients with Rectal Cancer of Different Ages 【Abstract】Objedtive To evaluate the feasibility of laparoscopic rectal resection in different ages patients with rectal cancer. Methods 128 patients with rectal cancer who were received operation therapy were included in this study. Older and younger patients were 64 case each, and 32 patients received laparoscopic rectal resection and 32 patients received open resection in either older and younger patients. Operation time, blood loss, ventilation time, hospitalization, proportion of postoperative analgesia, postoperative life self-care and complications were compared during different groups of patients. Results Except hospitalization, every other outcome measures in older patients by laparoscopic rectal resection were better than in older patients by open resection. blood loss, ventilation time, hospitalization, proportion of postoperative analgesia in younger patients by laparoscopic rectal resection were better than in younger patients by open resection. But there were no significant differences between younger patients by laparoscopic rectal resection and younger patients by open resection in postoperative life self-care and complications. Conclusion Laparoscopic rectal resection and open resection in older and younger patients were effective, safe and reliable. And older patients could get more benefits in laparoscopic rectal resection
