超声引导下经皮穿刺置管序贯治疗胰腺假性囊肿远期疗效观察 罗鸿昌1 孙珊珊1 张伟1 高孟1 刘若冰1 王敏2 李开艳1 武汉 华中科技大学同济医学院附属同济医院超声影像科 430030 武汉 华中科技大学同济医学院附属同济医院胆胰外科 430030 【摘要】 目的：探讨并总结超声引导下经皮穿刺置管序贯治疗胰腺假性囊肿的远期疗效。 方法：采用回顾性分析方法，对2012年1月至2014年6月我院收治经保守治疗无效的266例胰腺假性囊肿患者，其中234例患者行超声引导下经皮穿刺置管序贯治疗，32例患者外科手术治疗；统计并分析远期疗效及影响因素。 结果：266例患者共有298个胰腺假性囊肿，234例经皮穿刺置管序贯治疗的患者均成功完成超声引导下的置管引流，且未出现胰漏、感染扩散等并发症。225例患者治疗有效，囊腔闭合，囊肿消失，有效率96.2%。其中5名患者复发，复发率为2.3%。32例外科手术治疗患者，有效率96.7%。1例患者复发，复发率为3.1%。所有患者随访超过12个月。 结论：超声引导下经皮穿刺置管序贯治疗胰腺假性囊肿安全有效，具有微创、住院时间短及并发症少等优点，是一种值得临床推广的治疗手段。 【关键词】胰腺假性囊肿；超声引导；经皮引流；疗效 Long-term outcome in the percutaneous interventional treatment of pancreatuc pseudocysts guided by ultrasound 【Abstract】:Aims: To explore the application of percutaneous puncture and drainage guided by ultrasound in the treatment for pancreatic pseudocyst (PPC). Method: 234 Patients with PPC were treated with percutaneous puncture and drainage under B-ultrasound guidance. 32 patients with PPC was treated with surgery. All patients were recruited from January 2012 to June 2014. The long-term curative effect and influencing factors were analysised and statisticed retrospectivly. Result: 266 patients toally had 298 PPC. 234 patients were successfully completed under the guidance of ultrasound catheter drainage, and there was no complications such as pancreatic leakage, bleeding, infection and so on. 225 patients were treated effectively, closed the capsule, the cyst disappeared, and the effective rate was 96.2%. Among the 5 patients of recurrence, the recurrence rate was 2.3%.. The effective rate of surgical treatment was 96.7%., the recurrence tare was 3.1%. All patients were followed up for more than 12 months. Conclusion: With the advantages of minimally invasive, short hospitalization time and less complications, percutaneous puncture and drainage under B-ultrasound guidance in the treatment for PPC is safe and effective in the clinic treatment. 【Key words】: pancreatic pseudocyst; B-ultrasound guidance; percutaneous puncture
