2014年临床路径持续改进分析汇报? 问题背景：? 自2012年6月开始执行临床路径管理，最初入径病种12种，涉及8个科室，2013年全年临床路径总入径数：1158人次，完成人数879，中途退出人数279，入径率55.89%，入组完成率75.9%，好转治愈率75.82%，2014年1-6月份临床路径执行评价、效果评价持续下降（详见下图），因此由张守仁院长组织讨论整改此项问题。 问题：临床路径入径人数、入径率下降。 目标:提高临床路径执行率，使临床路径执行情况至少达成三级医院指标：入径率达50%，入组完成率70%。 寻找问题的原因： 一、信息数据搜集1、通过嘉和病历系统统计2013年及2014年上半年临床路径执行评价指标及效果评价指标的相关数据。 2.综合今创病案信息管理系统，提取临床路径经济学评价数据及执行效率评价指标数据。 （见附表：2014年1-6月临床路径统计表及2013年临床路径统计表） 二、召开相关职能科室及临床路径与单病种管理委员会成员会议 1、 通过各次会议，利用头脑风暴方法，收集临床路径问题出现的原因，问题解 决的瓶颈及应采取的整改措施。（详见各次会议记录摘要） 2、 通过会议决定调整临床路径与单病种管理委员会成员，由张守仁院长亲自把 关，任临床路径与单病种管理委员会组长，副组长：孙成宏院长，以及朱新兵、陈维峰、范律贤、田雷、庄玉芝、刘景兰为成员；并建立临床路径指导评价小组。（见南医政字【2014】31号发文） 三、将收集的数据进行系统的分析 1、 鱼骨图原因分析 ? 临床路径病 ? （英文版 ） easily blame, to prevent the broken window effect.
