Some Kleinians consider affect and the specific phantasy involved in PI as crucial elements to consider when formulating effective PI interpretations.

Herbert Rosenfeld (1952a) wrote of his work with psychoic patients who used PI as a way to grapple with severe paranoid-schizoid anxieties. Techniqucally, he believes in interpreting the primary phantasy associated with the anxiety. He writes of his work with one patient:

In describing his work with another patient, Rosenfeld (1954) states:
