中药复方治疗系统性红斑狼疮增效减毒的临床研究 【摘要】 目的 观察养阴清热中药复方治疗系统性红斑狼疮(SLE)的疗效。方法 采用随机、对照比较方法,将90例SLE患者分为中医组、中西医结合组、西医组,每组各30例,中医组予养阴清热中药复方,西医组予环磷酰胺(CTX),中西医结合组予中药复方联合CTX治疗,疗程6月,对患者临床表现及实验室指标进行观察。结果 总有效率分别为：中医组96.70%,中西医结合组93.33%,西医组80.00%(P＞0.05)。3组在治疗蛋白尿方面均有疗效(P＜0.05,P＜0.01),中西医结合组疗效优于西医组(P＜0.05)。中西医结合组及西医组升高白细胞作用优于中医组(P＜0.05)。3种疗法均有升高血红蛋白作用(P＜0.01)。中西医结合、中医、西医治疗均能降低血沉(P＜0.01,P＜0.05)。中西医结合治疗及中医治疗均能降低抗双链DNA抗体(P＜0.05)。中西医结合治疗能升高C4(P＜0.05)。结论 养阴清热中药复方具有免疫抑制作用,联合CTX治疗SLE能起到增效减毒的作用。 【关键词】 养阴清热组方；系统性红斑狼疮；增效减毒 Abstract：Objective To observe the effect about TCM of enriching yin and clearing heat in treating systemic lupus erythematosus (SLE). Methods It was a randomized, controlled trial. Ninety in-patients and out-patients with SLE were enrolled and divided into three groups. There were 30 patients in every group. One group was treated with TCM, one group was treated with cyclophosphamide, the other was treated with TCM combined with cyclophosphamide. At 6 months after treatment, the symptoms and relative indices were observed. Results The total effective rate was 96.7% in the TCM group, 93.33% in the TCM combined with cyclophosphamide group and 80% in the cyclophosphamide group (P＞0.05). Three groups all could improve total albuminuria (P＜0.05, P＜0.01). At improvement for total albuminuria, the curative effect of the TCM combined with cyclophosphamide group was better than the cyclophosphamide group (P＜0.05). At improvement for WBC, TCM combined with cyclophosphamide group and cyclophosphamide group were better than the TCM group (P＜0.05). Haemoglobin of three groups were all ascended (P＜0.01). Three groups all could decrease ESR (P＜0.01, P＜0.05). The antibody of dsDNA was decended after treatment of TCM combined with cyclophosphamide and TCM (P＜0.05). C4 was ascended after treatment of TCM combined with cyclophosphamide (P＜0.05). Conclusion TCM for nourishing yin and clearing heat can effectively regulate the immunologic function.
