急性上消化道出血急救指南 西南医院急救部 邓朝霞 加强治疗期2-10 诊断明确后的治疗与处理： 静脉曲张出血的治疗 非静脉曲张出血的治疗 药物治疗 首选：药物治疗。 推荐：生长抑素与抗菌药物联合治疗 首选：药物与内镜联合治疗。 推荐：PPI、生长抑素和抗菌药物的联合用药方法 内镜治疗 控制急性食管静脉曲张出血 方法：EVL、硬化剂治疗 推荐：生长抑素联合内镜治疗 上消化道出血的首选治疗 方法：药物局部注射、热凝止血、机械止血 介入治疗 适用于出血无法控制的患者 选择性血管造影 外科手术治疗 适用于出血不能控制或出血停止后24h内复发者 诊断明确但药物和介入治疗无效者，可考虑手术结合术中内镜止血治疗。 气囊压迫止血：仅作为过渡性疗法，以获得内镜或介入手术止血的时机 紧急治疗期 病因诊断期 加强治疗期 是 再次评估患者出血是否得到有效控制 考虑重复内镜治疗或手术 治疗原发病和随访 治疗后病情再次评估： 加强治疗期2-10 紧急治疗期 病因诊断期 加强治疗期 参考文献 1.
