三种方法治疗滋养细胞肿瘤的疗效评价 　　[摘要] 目的 探讨滋养细胞肿瘤的三种治疗方法的可行性及其疗效。 方法 选择从2011 年7月～2012年10月我院就诊被诊断绒癌及侵蚀性葡萄胎患者，根据其年龄、有无生育要求、病灶大小及肌层侵犯深度、治疗前血HCG水平、有无转移等因素分成三组；A组：射频消融联合化疗组，B组：子宫全切手术+化疗组，C组：单纯化疗组。对三组疗效进行评价。 结果 三组患者的治疗有效率分别为100.0%、91.7%、91.7%，三种治疗方法疗效之间差异无统计学意义（P>0.05）。 结论 化疗是滋养细胞肿瘤的首选及主要的治疗手段。对于较年轻、有生育要求患者射频消融联合化疗可以作为首选治疗方法。对于年龄较大、有转移并无生育要求患者来说子宫全切手术联合化疗是最好的选择。对于滋养细胞肿瘤患者采取以化疗为主，手术及自凝刀治疗为辅的个体化治疗手段，可获得满意的治疗效果。 　　[关键词] 滋养细胞肿瘤；化疗；子宫全切术；射频技术 　　[中图分类号] R737.33 [文献标识码] B [文章编号] 1673-9701（2015）23-0044-03 　　Efficacy evaluation of three methods in treatment of trophoblastic tumor 　　Shaadaiti?Wufuer Buhaiqiemu?Kadeer Gulina?Ababaikeli 　　Department of Gynecology， the First Affiliated Hospital of Xinjiang Medical University， Urumqi 830054， China 　　[Abstract] Objective To evaluate the feasibility and efficacy of three treatment methods for trophoblastic tumor. Methods The patients diagnosed with choriocarcinoma and invasive mole in our hospital from July 2011 to October 2012 were selected and divided into three groups according to their age， with or without fertility requirements， size of focus and invasive depth of muscular layer， pre-treatment blood HCG level， and with or without metastasis. Group A was radiofrequency ablation group， group B was hysterectomy+chemotherapy group， and group C was simple chemotherapy group. Efficacy of the three groups was evaluated. Results The effective treatment rates of the three groups were 100.0%， 91.7% and 91.7% respectively. The three treatment methods were not statistically different in the efficacy（P>0.05）. Conclusion Chemotherapy is the first choice and main treatment method of trophoblastic tumor. For younger patients with fertility requirements， radiofrequency ablation combined with chemotherapy can serve as the preferred treatment method. For elder patients with metastasis and without fertility requirements， hysterectomy combined with chemotherapy is the best choice.
