简易腹膜外剖宫产术的应用于再次剖宫产疗效观察 　　[摘要] 目的：探讨简易腹膜外剖宫产术应用于再次剖宫产是否有其优越性。方法：对2005年1月～2010年12月再次剖宫产病例进行回顾性分析。151例行简易腹膜外剖宫产术者为实验组，147例行新式剖宫产术者为对照组，对两组手术时间、术中术后出血量、术后胃肠功能恢复、术后病率、新生儿情况等进行对比观察。结果：两组在手术全程操作时间、术中出血量、术后病率、手术并发症、新生儿窒息率、切口愈合情况等方面比较，差异无统计学意义，但术后肛门排气时间实验组为（8.7±2.3） h，对照组为（17.4±3.9） h，两组比较，差异有统计学意义（P＜0.05）。结论：对于再次剖宫产患者而言，简易腹膜外剖宫产术较新式剖宫产具有术后胃肠功能恢复快等优点，适于临床推广应用。 　　[关键词] 简易腹膜外剖宫产；再次剖宫产；并发症；病例对照研究 　　[中图分类号] R719.8 [文献标识码] A [文章编号] 1673-7210（2011）06（c）-021-03 　　 　　Clinical observation of the simple extraperitioneal cesarean section in subsequent pregnancy 　　YANG Huaguang, LI Yi, HE Yurong, ZHAO Hongxi, YIN Guowu 　　Department of Gynecology and Obstetrics, Tangdu Hospital of the Fourth Military Medical University, Xi′an 710038, China 　　[Abstract] Objective: To study the application of the simple extraperitioneal cesarean section in subsequent pregnancy. Methods: A retrospective analysis was made to the cases of second cesarean section from January 2005 to December 2010. The observation group included 151 cases using the simple extraperitioneal cesarean section，and the control group included 147 cases using the new-style cesarean section. The time and bleeding in operation, illness and complications after operation, neonatal asphyxia and wound healing were compared in the two groups. Results: There was no significant difference between the time and bleeding in operation, illness and complications after operation, neonatal asphyxia and wound healing. The time of post-operative anus exhausting were (8.7±2.3) h and (17.4±3.9) h, respectively, and there was a significant difference (P0.05). Conclusion: For the subsequent pregnancy, the simple extraperitoneal cesarean section has many advantages over the new-style cesarean section, such as recovery of gastrointestinal function, and it is suitable for clinical application． 　　[Key words] Simple extraperitioncal cesarean section; Second cesarean section; Complications; Case-control study 　　 　　剖宫产、子宫肌瘤剔除术等导致的瘢痕子宫行再次剖宫产术在临床上屡见不鲜，不
