断指再植127例临床剖析 　　[摘要] 目的 探讨断指再植的手术治疗经验及对可能造成的失误原因采取相应的防治对策。方法 回顾性分析2009～2012年间我院治疗的断指再植患者127例共150指的临床资料，并对术后疗效进行评价，观察术后出现坏死及血管危象等情况。 结果 本组患者按照中华医学会断指再植功能评定试用标准，优80指，良52指，差18指，优良率达88%。术后出现血管危象22指，为14.7%。术后出现坏死共10例，成活率92.1%。 结论 影响断指术后成活的因素较多，临床医生应根据可能造成的失误原因采取相应防治对策，才能提高断指再植的成活率。 　　[关键词] 断指再植;血管危象;坏死 　　[中图分类号] R658.1 [文献标识码] B [文章编号] 2095-0616（2015）02-154-03 　　Clinical analysis of digital replantation in 127 cases 　　ZHANG Jian WAN Shengxiang XIAO Yingfeng 　　Department of Hand Surgery，Peking University Shenzhen Hospital，Shenzhen 518036，China 　　[Abstract] Objective To study the treatment experience of digital replantation and to take corresponding prevention countermeasures to the possible causes of the failure. Methods To retrospectively analyze the clinical data and to evaluate the postoperative curative effect of the 127 patients with finger replantation with 150 fingers cure in our hospital from 2009 to 2012.To observed the situation of necrosis and vascular crisis after operation. Results According to the evaluation trial standard of digital replantation function of Chinese Medical Association， the excellent and good rate was 88% with 80 excellent fingers，52 good fingers and 18 bad fingers.After the operation，the incidence rate of vascular crisis was 14.7% with 22 fingers，the survival rate was 92.1% with 10 necrosis. Conclusion Through the corresponding prevention countermeasures to the unsuccessful factors in major factors affecting of postoperative survival of digital replantation，to improve the survival rate of digital replantation. 　　[Key words] Digital replantation;Vascular crisis;Necrosis 　　断指再植是指将完全或不完全离断的指体在光学显微镜下利用显微外科手术技术进行彻底清创、血管吻合、骨骼固定、肌腱和神经修复，使其存活并最大限度地恢复其功能[1]。随着显微外科技术的不断发展，断指再植手术的成功率逐步升高。我科对2009～2012年127例断指再植术病历资料进行总结分析，手术成功率达92.1%，现将手术治疗经验及可能出现的失误及防治对策总结分析如下。 　　1 资料与方法 　　1.1 一般资料 　　选取2009～2012年间我院治疗的断指再植患者127例共150指为研究对象，所有患者均结合病史、临床表现、辅助检查确诊。其中男70例，女57例，年龄26～46岁，平均（35.7±4.2）岁。致伤原因：切割伤69例，电锯伤32例，挤压伤26例。伤残指体：拇指58指，示指37指，中指31指，无名指和小指12指。完全离断
