安徽医 安徽医科大学硕士学位论文 PAGE PAGE 5 The surgical therapy of involve weight bearing area acetabular fractures and analysis of the factors of influence results Abstract Objective This paper retrospectively reviewed the clinical diagnosis and surgical treatment of involving weight bearing area acetabular fractures, To explore the factors that affect the efficacy, and look forward to improve the clinical efficacy. Methods 16 cases of surgical treatment inpatients with involving the weight bearing area acetabular fractures in our in our department in June 2008 ~June 2012 have been selected.Before surgery, we take three standard iconography inspections ,including Judet X-ray,anteroposterior hip obturator oblique,iliac oblique included，CT scan and three dimensional reconstruction.By Letournel-Judet hip acetabular fracture taxonomies classification，16 cases of involving weight bearing area acetabular fracture with dislocation were treated with open reduction and internal fixation with reconstruction plates，K-L approach was adopted in 10 cases,ilio inguinal approach in 1 cases,and anterior combined posterior approach in 5 case.Reviewing digital X-ray one week later after surgery to observe the result of fracture reduction and wound healing.Patients followed up have been carried out including physical examination, X-ray and CT scan so as to proceed statistics of post-operative complications. Results 16 cases were followed up for 6~48 months.All fractures healed in 3~5 months.Fractures reduction quality were assessed by Matta radiographic grade: anatomic reduction 9 cases，satisfaction reduction 6 cases，dissatisfaction reduction 1 cases.Function of hip joint were assessed by D’Aubigne-Postel，and the following results :5 cases were excellent，8 good，2 fair，1 false.Traumatic arthritis was found in 3 cases，heterotopic ossification in 1 cases.No infection，sciatic nerve injury and femoral head necrosis were found..
