最新. 最新. * Prevention of incidents, develop and implement process for safety management, analyze safety issues and trends , investigate incidents and use root cause analysis methodologies, develop safety recommendations, follow through on recommendations. Not just implement human factors training or awareness training but strategies such as collecting data and analyzing this data to look for trends where errors and mistakes are made and can be avoided or corrected. We have come a long way since 1988 but we still have more to do. Together we can accomplish goals to reduce human error and enhance human performance to reduce the accident and incident rate contributable to humans in the system. ? I would like to point out a few recent facts reported by a study done by USA today reported on March 13, which looked at 14 years of FAA data. Maybe some of you read the news report. ? ·??? Airlines were fined more than 9.4 million in safety regulation violations ? ·??? An average of 8 times each day FAA fined or took administrative action for safety violations ? ·??? Maintenance and security accounted for 3 quarters of these fines from 1985-1998 ? ·??? Maintenance accounted for 38% !! Some of these violations were because of not performing the required inspections, paperwork not being completed or completed incorrectly, not complying with maintenance orders, improperly operating aircraft in altered state, delaying repairs beyond time limits. These are just a few examples. Some of these examples such as paperwork errors may not mean that the aircraft is not airworthy or an immediate safety issues but administrative action or fines were levied. These statistics end in 1998. 1999 was not a great year for aviation’s safety record. As more passenger miles were flown in 1999 the incident and accident rate has gone up. We can not allow this to continue. We must do a better job both in industry and at the FAA. ? ·??? Communication, whether written or oral must be corre
