Sometimes patients may have a strong desire for such treatment, because it is a temporary measure potentially leading to the restoration of health. At other times, such treatment may be undesirable, because it may only prolong the process of dying rather than restore the patient to an acceptable quality of life. In any case, each person is seen, under the law, as having the personal right to decide whether to continue or terminate such treatment. As long as a patient is mentally competent, he or she can be consulted about desired treatment. However, when a patient has lost the capacity to communicate the situation is different and that is what makes a living will necessary.

Name of a guardian for minor children
Name of an alternative guardian, in the event your first
choice is unable or unwilling to act
