隆鼻术中鼻背增高程度与假体厚度的相关性分析 吴华1 吴溯帆1 石杭燕1 严晟1 徐舒畅2 廖盛辉2 王振3 第一作者：吴华 1982- 湖北人 硕士 住院医师 通讯作者：吴溯帆 浙江人 教授 主任医师 作者单位：1.浙江省人民医院整形外科 2.浙江大学计算机科学与技术学院 3.浙江省人民医院放射科 本项目为浙江省科技厅重大科技项目，项目名称：《基于虚拟人技术的三维整形设备关键技术研究及开发》，项目编号：2008C01048-2 【摘要】　目的 精细测量隆鼻术后鼻背的增高程度及植入体的厚度，并分析两者之间的相关性，为临床医生对假体的设计和雕刻提供理论依据。方法 对20例单纯隆鼻的求美者行手术前后的CT扫描，使用自行研发的软件对CT扫描数据进行处理，重构患者面部三维立体模型,并在三维模型上精确测量鼻根点、鼻骨下点及鼻尖点在手术前后的高度变化，将高度的增加值与相应部位的植入体厚度进行比较，并对相关数据进行计算及分析。结果 隆鼻术后鼻背增高的平均值为鼻根点4.085±0.79mm、鼻骨下点3.85±0.78mm、鼻尖点2.29±0.42mm，鼻根点与鼻骨下点术后增高值与植入体的厚度相近，而鼻尖点的增高值仅为该部位假体厚度的69.29%。结论 在鼻的不同部位，植入体的厚度对鼻背高度的补充效率是不一样的，临床上在假体制作时应充分考虑到这一特点，以期达到最佳的隆鼻效果。 【关键词】　隆鼻术 人体测量　计算机 CT扫描 三维重建 The analysis of dorsal elevation value and the prosthesis thickness in the augmentation rhinoplasty 【Abstract】　Objective To measure the elevation value of nasal dorsum and the thickness of prosthesis in the augmentation rhinoplasty, and calculate the ratio between them, to provide the theoretical parameters for the clinical design and sculpture of prosthesis. Methods Twenty objects of augmentation rhinoplasty received cranial CT scanning before and after operation, the data of which was analyzed using developed software. The three-dimensional facial models were rebuilt and accurate measurement was performed by the computer. The increased value of nasal dorsum at the nasion point (NP), under point of nasal (UP) and pronasale point (PP) was calculated and compared to the thickness of the prosthesis. Results The average increased value of nasal dorsum at NP, UP and PP were 4.085±0.79mm, 3.85±0.78mm and 2.29±0.42mm, respectively. At NP and UP, the dorsal elevation values were closed to the thickness of prosthesis, whereas at the PP, the dorsal elevation value was only 69.29% of the thickness of the prosthesis. Conclusion The ratios between the dorsal elevation value and the thickness of prosthesis varied at the different parts of the nasal dorsum, which should be considered in the clinical practice to achieve the expected result. 【Key words】 augmentation rhinopla
