羊痘病例報告 疫學研究組 李淑慧副所長兼疫學研究組組長 張仁杰助理研究員 摘 要 2008年6月，桃園縣羊，臨床上顯現發燒、流鼻汁、眼鼻分泌物增加、結膜炎及呼吸困難、乳房炎、流產及嚴重皮膚病變死亡率可達以上。全身出現硬實突起結節，(marginated chromatin)，呈孔洞狀空泡化病變；肺臟呈增殖性及間質性肺炎。肺、皮膚節結及粘膜病灶區乳劑以負染色法鏡檢，可見病毒型態為磚型表面具有管狀小體構成絲狀結構痘病毒綿羊/山羊痘病毒聚合酶鏈鎖反應（PCR），PCR產物進行序列分析，與山羊痘病毒相似性為99%臨床症狀、肉眼病變、病理學、電子顯微鏡PCR及核酸定序等結果，確診為山羊痘病例。 The first oat pox outbreak in TaiwanAnimal Health Research Institute, Council of Agriculture, Executive Yuan Jen-Chieh Chang and Shu-Hwae Lee Abstract Goats on two farms in Taoyuan County showed clinical signs of extremely thin, fever, serous nasal discharge, lacrimation, conjunctivitis and dyspnea, mastitis, abortion and severe skin lesions. The high mortality (up to 50 percentage) and gross findings was noted. Laboratory tests on June 27 confirmed goat pox broke out on these farms. There were hard nodules and varying degrees of papule including small erythematous macule, papules, round ulceration and crust cover the whole body, especially in the back, neck, shoulder, mouth, lips, face, inside ear and skin of udders. Histopathological examination revealed significant skin dermatitis, vasculitis and local inflammation of corium. The nuclei of infected cells were vacuolated and have marginated chromatin with vacuolated cytoplasm contained single or multiple eosinophilic intracytoplasmic inclusion bodies. The pulmonary lesions were proliferative, interstitial pneumonia and bronchiolitis. The trachea and nasal mucosa showed multifocal necrosis with epithelial cells degeneration, necrosis and ulceration. Severe congestion and hemorrhagic lesions were noted in the trachea and nasal submucosa. The emulsion material was also tested by negative staining under the electron microscope. The pox virions were brick-shaped and cover in shorted tubular elements. The extraction of nucleic acid also amplified with specific sheep / goat pox virus primer by polymerase chain reaction (PCR) and the PCR products were analyzed by sequencing.
