血细胞分离机AutoPBSC程序与MNC程序 采集外周血干细胞的效果比较及影响因素分析 曾丰1魏世金1黄豪博1黄清华1林秋燕1范丽萍1黄惠炆1付丹晖1，2* 1福建医科大学附属协和医院输血科；2福建省血液病研究所血液科，福建福州350001 摘要： 本研究旨在分析COBE Spectra血细胞分离机自动采集程序（AutoPBSC程序）与半自动采集程序（MNC程序）在外周血干细胞采集中的效果及影响因素。109例采集按对象不同分为患者自体组（患者）和异基因供者组（供者），通过比较采集物中干细胞数量与质量及采集程序特点，分别对两种采集程序在两组中的采集结果及影响因素进行分析。结果表明在全血处理量基本相同的情况下，全组中两种程序采集物中MNC%与CD34+%、CD34+细胞数及血红蛋白含量差异无显著性（P＞0.05）；与MNC程序比较， AutoPBSC程序采集物中血小板混入少，采集物体积小，但抗凝剂用量多，采集时间长（P＜0.05）。相关性分析显示：患者组两种程序采集物中MNC数（r=0.314,P=0.015）、CD34+细胞数（r=0.922,P=0.000）与采集前对应参数呈正相关，采集物CD34+细胞数与WBC数（r=0.369,P=0.004）及MNC数（r=0.495,P=0.000）正相关；供者组Auto PBSC程序采集物中MNC数与采集前呈正相关（r=0.896,P=0.000），MNC程序采集物中CD34+细胞数与采集前呈正相关（r=0.666,P=0.000）。患者组Auto PBSC程序采集物中MNC数与CD34+细胞数男性显著高于女性（P＜0.05），年龄40岁以下显著高于年龄40岁以上（P＜0.05），而MNC程序年龄40岁以上患者采集的CD34+细胞数显著高于年龄40岁以下患者（P＜0.05）。供者组仅MNC程序采集物中MNC数与CD34+细胞数男性显著高于女性（P＜0.05）。结论：在全血处理量相同时，患者自体和健康供者PBSC采集中两种程序收获的MNC纯度与CD34+细胞纯度及浓度高度一致但患者自体PBSC采集结果受年龄与性别影响较大。 关键词：外周血造血干细胞；采集；自体；异基因；AutoPBSC程序；MNC程序 Analysis of the Efficiency and Influence Factors of PBSC Collection with Cell Separator ：comparison between AutoPBSC and MNC procedure Abstract This study was aimed to analyze efficiency and influence factors of PBSC collection by an automatic (AutoPBSC procedure) and a semiautomatic apheresis procedure ( MNC procedure) of COBE Spectra cell separators. According to the different objects, 109 cases of apheresis were divided into autologous cohort (patient) and allogeneic cohort (donor), we made a comparison of the qunatity and quality of the collections and the characteristics of apheresis procedure, analysed the yields and influence factors of two cohorts with two kinds of procedures respectively.
