摘 要 基本医疗保险是社会保障制度的一个重要内容，它是维护社会稳定的一个重要的安全网。随着我国经济的飞速发展，人民对健康的要求越来越高，与此相适应，国家正在积极推进基本医疗保险的改革，人民群众的基本医疗需求得到了一定程度的满足。医疗卫生体制在不断完善，经济也在不断发展，虽然卫生服务的效率大幅度提高，但与此同时，收入分配制度的不尽完善扩大了社会成员间的贫富差距，这会导致卫生服务利用的不公平性。不公平性的出现极大地影响了社会的和谐。这就是现阶段我国进行医疗保险公平性研究的客观必要性。 本文对公平性的研究主要从两个方面展开：一是当代人的横向公平性研究，文章以山东省临沂市为例，通过对参保公平性、筹资公平性、医疗服务利用公平性等的问题分析，提出了临沂市医疗保险公平性存在的问题；二是代际间的纵向公平性研究，文章以陕西省为例，指出了人口老龄化对医疗保险基金正常运营带来了压力。在我国的医疗保险制度中，退休人员不缴费，这更进一步给基金的平衡运作带来了风险。本文正是在这样的背景下，以陕西省为例，建立了基金收支平衡预测模型，从而论述了代际间不公平行的存在。 最后，在建立一系列实证分析、模型建立分析的基础上，文章提出了优化医疗保险公平性的相关对策建议。 关键词：医疗保险，公平性，基尼系数，老龄化 ABSTRACT The basic medical insurance system is an important content in a social security systems, it maintains the social stability safety. With the rapid development of China’economy, people’demand for health become higher and higher. In order to catch this requirement, the state is actively carrying out Medical insurance system reform, and this meet the requirement of medical and health to some extent. Health system continues to improve, economic is continuing growing, these improve the efficiency of healthy services, but at the same time, the expanded gap between rich and poor be caused by the different income will be able to cause the hygienic service use inequity. And this is the necessity of the insurance fairness research in our country. The study for the fairness from two aspects: the first one, the horizontal equity of contemporary human, take Linyi City, Shandong Province as an example. Through the equity in service and so on, the passage point out there are some health insurance equity problems; the second one, longitudinal intergenerational equity research, the passage take Shanxi Province as an example, point out that the population aging has put pressure on health insurance fund’s normal operations. In China’s Medical insurance system, the retirees needn’t to pay for their medical care, and this bring risk to the balance of fund further operation.
