河南省拟推荐2015年度国家科学技术奖项目 一、自然科学奖1项： 项目名称：食管鳞状细胞癌遗传易感性和癌变多阶段演进分子机制 推荐单位：河南省 学科评审组：基础医学评审组 项目简介： 中国是世界上食管癌发病率和死亡率最高的国家，全球每年新增约50万食管癌患者中，一半以上发生在中国。显著的地域性分布差异（高、低发区发病率可相差500倍）和明显的家族聚集性发生（家族史阳性患者占40%）是食管癌突出流行病学特征，提示环境和遗传因素在食管癌发生中起重要作用。本研究团队正是围绕阐明食管鳞癌多阶段演进和遗传易感性分子机制的研究目标，从1995年至今走访12万个自然村，400家医院，建立41.4206万例食管癌临床信息和生物样品资料库，结合现代分子生物学技术，旨在甄别食管鳞癌关键易感基因和食管癌变早期分子变化特征和规律，为建立用于大规模高危人群筛查、早期发现和个体化防治提供理论基础和分子靶标。 本项目取得的创新性成果包括： 发现2个中国人食管鳞癌重要易感基因：PLCE1和C20orf54基因（又称核黄素转运基因，RFT2）； 发现核黄素缺乏和亚硝胺二种食管癌发病因素的遗传分子新机制，提出食管癌个体化预防新观点； 发现p53-Rb信号传导通路是中国人食管鳞癌癌变多阶段演进的关键分子改变，并可在外周血中检测到； 阐明食管上皮癌变早期上皮细胞过度增生的形态学变化和分子变化的关系，为食管癌早期发现、食管癌前病变干预效评价及食管癌变多中心起源理论等提供了重要分子依据； 建立食管上皮癌前病变组织细胞增生H3-TdR标记和定量分析方法。 综上，本研究成果为建立食管癌高危人群预警和个体化防治分子靶标和手段，进一步阐明环境和遗传因素交互作用对食管癌发生的分子机制等方面提供新的研究思路和方向。 主要论文目录： Genome-wide association study of esophageal squamous cell carcinoma in Chinese subjects identifies susceptibility loci at PLCE1 and C20orf54.
