广西药品和医疗器械集中采购 信息反馈系统登陆界面： 用户注册界面： 注册时，必须如实填写企业名称、被授人姓名、身份证号、联系电话等，企业名称不能填写简称。 填写质疑和申诉：登陆系统后，点击左边【质疑和申诉】菜单，打开质疑和申诉填写界面，如下图： 【质疑和申诉】菜单： 填写申诉内容： 申诉各项目必须如实填写，药品名、剂型、规格严格按生产批件上的名称填写，企业名称严格按营业执照上的名称填写，不得使用简称。 3、提交和上报申诉：按所列项目填写完申诉产品和申诉内容后点击【提交】，在申诉列表可看到所提交的申诉产品，确认申诉各项内容无误后，点击【上报】，即可将该申诉产品上报采购管理中心，上报后该申诉产品的各项内容将不能再做修改和删除，上报前该申诉产品的各项内容均可做修改和删除，如下图： 上报申诉： 4、打印和导出申诉产品列表：在申诉产品列表下有【打印】和【导出】按钮，如上图，点击【导出】，即可将上报的申诉产品导出成EXCEL表格，导出后注意将导出文件另存为电子表格式，点击【打印】可将上报的申诉产品列表打印出来，如下图： 未上报的申诉产品将不能导出和打印。打印出来的申诉表格，请注明标书序号、生产商名称、被授权人名称和联系电话，盖上公章，递交到广西药械集中采购管理中心。 5、修改个人资料和退出系统：点击【修改个人资料】菜单可以修改自己的注册资料，点击【退出系统】，退出该信息反馈系统，如下图： 广西壮族自治区药品和器械集中采购管理中心 技术科 2009年3月30日 -------------------------------------------------- button.
