车祸伤患者心理应激状态及影响因素分析 孙溦 汪涛 屈纪富 文亮 刘明华 王玉姝 第三军医大学附属西南医院急救部 400038 摘要：目的：了解车祸致伤患者心理应激状态以及相关的影响因素。方法：利用SCL－90量表、自制伤员信息表、创伤严重度评分（ISS）对因车祸致伤入院的伤员进行心理健康状态与相关因素调查。将结果利用SPSS13.0软件包进行统计分析。结果：车祸伤患者SCL－90总均分及各因子得分高于常模，P0.01其结果显著。其中SCL－90各因子得分增高与患者的性别、既往身体健康状态、社会交往支持和创伤严重程度有明显的相性关，p0.05～0.01。结论：车祸伤患者在遭受突然的车祸打击后有较高的心理应激反应，这种心理应激与伤者的性别、既往身体健康状况、社会支持和创伤严重度有明显的相关性；提示车祸伤患者伤后心理应激反应较高，其心理健康状况不容乐观，在重视车祸伤患者的急救与治疗同时切不可忽略伤者的心理状态，早期发现不良或过度的心理应激与及时的给予心理干预是救治车祸伤患者使其能够有完整的身心康复、增强其社会适应能力且是不可忽略的重要措施。 Analysis of stress mentalities of traffic patients and their effecting factors Sunwei wangtao qujifu wenliang liuminghua wangyushu Department of Emergency Medicine, Southwest Hospital, Third Military Medical University, Chongqing, 400038 Abstract: objective: to understand the stress mentality of traffic patients and its effceting factors. Results: the total values and every value in SCL-90 table of traffic patients were significantly higher than those of healthy control (P0.01). the increase of value in SCL-90 were significantly correlated to gender, historical health, social intercourse and injury severity. Conclusion: there are strong stress mentalities in patients with sudden traffic accident, which are significantly correlated to gender, historical health, social intercourse and injury severity. We should not ignore the disorder mentalities of traffic patients during the resuscitation and treatment. It is very important to give early mental intervention to prevent traffic patients from distress or hyperirritability so as to recover physically and mentally and facilitate social adaptabilities.
