文献标识码：A 第 25 卷第 28 期 2015 年 10 月 中国现代医学杂志 China Journal of Modern Medicine  Vol. 25 No.28 Oct. 2015  文章编号： 1005-8982（2015）28-0036-05 ·临床论著· 尿足细胞标志蛋白 podocalyxin 在肾病综合征 合并急性肾损伤诊断中的价值 王福珍 1，丘昭文 1，朱杰 2，吴森超 1，林冲云 1，王朝晖 3 （1 .福建医科大学附属龙岩市第一医院 肾内科，福建 龙岩 364000；2.上海师范大学数理学院， 上海 200234；3.上海交通大学医学院附属瑞金医院 肾内科，上海 200025） 摘要：目的 探讨原发性肾病综合征（PNS）患者尿液中 podocalyxin（PCX）的变化对急性肾损伤（AKI）的 诊断价值。方法 对 PNS 合 AKI 患者（n =33 例，AKI 组）和未合并 AKI 患者（n =24 例，非 AKI 组）尿液 PCX、 血清半胱氨酸蛋白酶抑制物 C（Cystatin C，Cys C）、血清低密度脂蛋白胆固醇（LDL-C）和血浆清蛋白（ALB） 水平进行检测，观察上述指标与病情关系。结果 AKI 组尿 PCX （42.04±11.01）ng/ml 显著高于非 AKI 组 （30.88±9.93）ng/ml；血清 Cys C 水平（1.91±0.54）mg/L 显著高于非 AKI 组（1.29±0.34）mg/L，而血清 LDL-C 和血浆 ALB 水平在两组中比较差异无统计学意义。随着 AKI 病情加重，估算的肾小球滤过率（eGFR）的减少， 尿 PCX、血清 Cys C 增加更为明显。结论 联合检测原发性肾病综合征患者尿 PCX、血清 Cys C 有助于 AKI 的诊断及病情判断。 关键词：尿液；podocalyxin；肾病综合征；急性肾损伤 中图分类号：R692 Urinary podocalyxin as a marker of primary nephrotic syndrome complicated with acute kidney injury Fu-zhen WANG1, Zhao-wen QIU1, Jie ZHU2, Sen-chao WU1, Chong-yun LIN1, Chao-hui WANG3 (1.Department of Nephrology, the First Hospital of Lonyan, Fujian Medical University, Longyan, Fujian 364000, P.R.China; 2.
