PAGE ①湖南省人民医院（湖南师范大学第一附属医院）乳甲外科 湖南长沙　410005 超声引导下乳腺微创旋切术与开放手术在治疗乳腺良性肿块的对比研究 何杰 张超杰 范培芝 谢靖 (湖南省人民医院乳甲外科 湖南 长沙 410005） 摘要：目的 探讨在B超引导下行乳腺良性肿块微创旋切术与传统开放手术的疗效对比。方法 回顾性分析3年来在局麻下行乳腺良性肿块手术治疗患者250例，分别采用微创旋切术（135例）和传统开放手术（115例），比较两种术式的手术时间、术中出血量、切口长度、患者满意度及术后并发症的发生情况。结果 微创旋切术在手术时间、术中出血量、切口长度及患者满意度上较传统手术有不可比拟的优势（P均0.05），而在术后并发症上无明显差异（P均0.05），所有患者无一例出现感染及术后血肿。术后随访3~12个月，均未发现乳腺内残留病灶及肿块复发。结论 超声引导下的乳腺微创旋切术较传统开放手术有定位准确，时间短，出血量少，术后恢复快，并发症少，切口美观的特点，是年轻女性，特别是多发性乳腺肿块患者的首选治疗方法。 关键词：乳腺肿块；Mammotome；微创旋切术；传统开放手术；对比研究 中图分类号： 　文献标识码：　文章编号： The Comparative Study of The Efficacy of B Ultrasonic-guided Minimally invasive biopsy in the Removal of Benign Breast Lumps with Open Operation Abstract:Objective: A comparative study of the efficacy of two types of benign breast mass surgery, the B ultrasonic-guided minimally invasive biopsy and the traditional open surgery. Methods: A retrospective analysis of benign breast mass surgery under local anesthesia was performed in 250 surgical patients for three years including the B ultrasonic-guided minimally invasive biopsy (135 cases) and the traditional open surgery (115 cases). The operative time, blood loss, incision length, patient satisfaction and postoperative complications were compared between the two methods. Results: we found that the minimally invasive biopsy surgery significantly outperformed (p0.05) the traditional method in terms of operation time, blood loss, length of incision and patient satisfaction. Importantly, the minimally invasive biopsy showed a comparable level of postoperative complications with the traditional method (p0.05), no cases of infection and postoperative hematoma were detected. All patients were followed up for 3 to 12 months and no residual tumor and recurrence of breast tumor were found. Conclusion: the B ultrasonic-guided minimally invasive biopsy exhibits many advantages over the traditional open surgery method including the accuracy of positioning, short time, less bleeding, rapid postoperative recovery,
