58-5车体升降机火情报告#58-5 VDL fire incident preliminary reportMFG SAFETY2014.06.14背景介绍：58-5号车体升降机用途为从二楼电泳烘炉出口将车体垂直降至一楼。整个升降机封闭于电泳烘炉末端。Background: #58-5 VDL is used for transferring unit from exit of ED oven located on 2nd floor to 1st floor. The VDL located at the end of ED oven.事故地点：CAF1 涂装车间B线电泳烘炉出口Incident location: exit of B line ED oven, paint shop, CAF1 事故时间：6月14日7:20左右Incident time: 7:20 am, Jun. 14th 事故描述：维修人员在监控电脑上发现58-5号车体升降机滚床变频器故障报警。随即6名维修人员赶至现场处理。发现升降机滚床支撑架转折处有小团明火。在扑灭前火情持续20多分钟。Incident description: at above time and date, maintenance found out malfunction alarm for #58-5 VDL conveyor. Then 6 maintenance persons responded to the scene and a small flame was observed at the corner of conveyor support frame. Flame continued 20+ minutes before extinguished.应急措施 immediate actions：1. 使用灭火器和消防水龙带灭火；1. extinguish flame with extinguishers and fire hoses,2. 关断所有相关电源、风机、风阀并通知PE断开T17变电站开关；2. shut off all relevant power, fan, fan valve and notice PE to shut off T17 substation power,3. 通知ERT，通知119，消防队抵达现场后发现火情已被扑灭；3. Notice ERT, call 119, out-source fire fighter team responded to the scene and found out flame had been already extinguished.4. 通知制造部李建平总监，涂装车间唐鸿艳经理等管理层和安全部；4. Notice MFG director Li Jianping, paint shop area manager Tang Hongyan and safety. 临时措施 ICA：1. 在安全监督下有序组织维修升降机，恢复生产。预计19:00前受影响B线恢复生产；1. Arrange VDL maintenance to recover production under safety supervision. It’s estimated that affected B line full recovery before 19:00.2. 根据现场情况重新组织工厂生产：停止焊装CD线白班生产；涂装除前处理电泳B线外，前处理电泳A线、密封胶线、中面涂线均维持生产；总装、发动机维持生产，取消白班2小时加班；2. Re-organize plant production according to onsite status: cease body CD line day shift production; maintain pre-treatment A line, seal, primary top coat production line running except pre-treatment B line; maintain TCF and engine shop production running, cancel 2 hours overtime for day shift, 3. 通知升降机厂商大福技术人员到现场协助调查；3. Ask OEM DAIFUKU engineer arrived at the scene to involve investigation,4. 成立事故调查组；4. Incident investigation team established,事故调查组
