胎膜早破阴道分泌物异常对母婴结局的影响 (深圳市第六人民医院妇产科) 摘要：目的探讨阴道分泌物对胎膜早破母婴结局的影响。方法将450例胎膜早破(PR0M)患者分为阴道分泌物正常组和异常组，分析比较阴道分泌物结果对母婴结局的影响。结果PROM阴道分泌物检查异常率达65．78％(296／450)。PROM阴道分泌物异常组期待治疗时间短，胎儿窘迫、新生儿窒息、围产儿死亡、绒毛膜羊膜炎、剖宫产率均较阴道分泌物正常组升高，两组间差异有显著性(P0．05)。而经过积极抗感染治疗后，两组新生儿感染率和产褥病率比较无显著性差异(P0．05)。结论PROM与生殖道上行感染密切相关。孕前及孕期作阴道分泌物检查、治疗阴道炎症可降低胎膜早破的发生率，减少孕产妇和围生儿并发症。 The influence of the vagii~ secretions Oil PROM and the maternal-infant outcome RAO Yun，LI I， 伽g．(Department ofOb— s~trics and Gynecology，The sixth people’S hospital ofShenzhen，Shengzhen，518052，China) Abstract：Objective To investigate the influence of the vaginal secretions on PROM and the maternal—infant outcome．Methods 450 patients with PROM were divided into normal vaginal secretion group and abnormal va舀 secretion group．The matemal-infant outcomes were analyzed and compared between two groups．Results The abnorm al rate ofvaginal secretions on PROM was about 65． 78％(296／450)．Compared with normal vaginal secretions group，the expectation therapy time Was shortened and the incidence rates of fetaldistress，asphyxia neonatorum，neonate death，ehofioamnionitis，caesarean section were increased obviously(P0．05)．With effeetive anti-infection therapy，the newboiTl and mother infection incidences had no signiflqcant diference between two groups．Conclusion Th ere is a close correlation between PROM and reproductive tract infection．Vaginitis should be detectde and cured before and among pregnancy，SO as to prevent PROM，at the sanle time，reduce the complications of maternity and fetus． Key words：Premature rupture of tl1e membrane；Vaginal secretions；Infection；Maternal—infant outcome (Chin J Lab D／agn，2011，15：1876) 临产前胎膜破裂称为胎膜早破(premature mp—ture of the membrane，PROM)，是产科的一种常见并发症，发生率为5％ 一10％_l J，可诱发早产、感染、胎儿窘迫，增加孕产妇和围产儿的患病率和死亡率。有研究表明，孕妇生殖道感染与胎膜早破的发生密切相关。本文对450例胎膜早破产妇阴道分泌物结果进行回顾性分析，旨在探讨阴道分泌物异常与胎膜早破母婴预后的关系。 1 资料和方法 1．1 一般资料 分析我院2008年10月一2009年11月住院分娩的胎膜早破孕妇450例，年龄l9—40岁，孕周28— 4l周，无内外科合并症及其他产科并发症。据入院阴道分泌物检测结果分为正常组(154例)，异常组(296例)。正常组年龄(25．33±1．16)岁，入院时孕龄(34．59±1
