胫后动脉穿支皮瓣的临床应用 南华大学附属南华医院手外科（湖南 衡阳 421002） 夏晓丹 唐举玉 谢松林 刘鸣江 刘建书 【摘 要】 目的 介绍胫后动脉穿支皮瓣在小腿及足、踝部软组织缺损修复中的应用。方法 2007年6月~2008年11月，应用带蒂的胫后动脉穿支皮瓣修复软组织缺损13例。其中小腿下段3例，足跟5例，足背2例，内踝3例。结果 所有皮瓣均完全存活，随访3~12个月，各皮瓣质地柔软，皮瓣不臃肿，外形美观,供区植皮均成活，无并发症。结论 胫后动脉穿支皮瓣能以最小的供瓣区代价达到高效的修复效果，是一种修复小腿及足、踝部软组织缺损的理想术式。 【关键词】胫后动脉、 穿支皮瓣、 软组织缺损、 修复 中图分类号：R622.1 R616.2 文献标志码：B APPLICATION OF POSTERIOR TIBIAL ARTERY PERFORATOR FLAP (Nanhua Affiliated Hospitalof Nanhua University, Hengyang 421002, Hunan, China) Xia Xiaodan Tang Juyu Xie Songlin Liu Mingjiang Liu Jianshu 【Abstract】 Objective To recommend the application of posterior tibial artery perforator flaps for repairing soft-tissue defects of lower leg,ankle and foot. Method From June 2007 to November 2006,13 pedicled perforator flaps were used to repair the soft-tissue defects of lower leg, ankle and foot. Among them, 5 cases of soft-tissue defects of heel, 2 cases of soft-tissue defects of instep, 3 cases of soft-tissue defects of medial ankle, 3 cases of soft-tissue defects of ventral leg. Results All the flaps were alive. The follow-up for 3-12 months revealed that these flaps were soft and complanate, with good shape. There are no complications in the donor sit. Conclusion The posterior tibial atery perforator flap is a perfect method to repair soft-tissue defect of lower leg, ankle and foot because of the minimal donor site “cost” and the maximal efficacy. 【Key words】Posterior tibial artery 、 Perforator flap 、 Soft-tissue defect Repair 穿支皮瓣是指管径细小(0.5~0.8 mm)的皮肤穿支血管供血的皮瓣，属轴型血管的皮瓣范畴，是显微外科皮瓣移植的新发展[1.4]，自1989年Koshim等提出穿支皮瓣的概念以来，因其符合组织移植“受区修复重建好，供区破坏损失小[1.4]”的原则在临床上得到广泛应用。2007年6月~2008年11月，我科应用了胫后动脉穿支皮瓣移位修复小腿及足、踝部软组织缺损13例，获得了满意效果，现报告如下。 1 临床资料 1.1 一般资料 本组13例，男9例，女4例。年龄16~45岁，平均31.2岁。其中应用修复小腿下段软组织缺损3例，足跟软组织缺损5例，足背软组织缺损2例，内踝软组织缺损3例。致伤原因：车轮碾压伤4例，石块等重物砸伤4例，机器绞压伤2例，胫骨下段骨折术后钢板外露3例。皮瓣切取面积5 cm×3 cm 至15 cm×6 cm。均采用带蒂转移的方法。 1.2 手术方法 术前均用彩色超声多普勒沿胫后动脉逐一标记出各个穿支穿出点位置，根据创面位置选择合适的一根穿支为营养血管，该穿出点即为皮瓣旋转点，并用标记笔标识。术中先切开一侧皮肤，在浅筋膜层小心分离，将皮肤牵
