系统性红斑狼疮伴中重度血小板减少的临床特征 　　【摘 要】目的：探讨系统性红斑狼疮伴中重度血小板减少患者的临床特征。方法：收集20例系统性红斑狼疮伴中重度血小板减少患者的临床资料，并与20例血小板正常的系统性红斑狼疮患者对比，比较两组患者实验室检查结果、临床症状和体征及疾病活动度的差异。结果：两组患者?在发热、皮疹、关节炎等临床症状和体征，抗ds-DNA、抗NUC、抗HIS、抗U1RNP、抗SSA、抗SSB、抗C1q抗体阳性率及疾病活动度等方面比较，差异无统计学意义（P 0.05）；但系统性红斑狼疮伴中重度血小板减少患者白细胞计数、血红蛋白浓度及补体C3水平更低，抗磷脂抗体阳性率更高，差异有统计学意义（P 0.05）。结论：系统性红斑狼疮伴中重度血小板减少患者较系统性红斑狼疮血小板正常患者?白细胞计数、血红蛋白浓度及补体C3水平更低，?抗磷脂抗体阳性率更高。 　　【关键词】 红斑狼疮，系统性；血小板减少；抗血小板抗体 　　doi：10.3969/j.issn.2095-4174.2015.08.004 　　【ABSTRACT】Objective：To investigate the clinical characteristics of systemic lupus erythematosus with moderate or severe thrombocytopenia.Methods：Clinical data of 20 cases of systemic lupus erythematosus with moderate or severe thrombocytopenia were collected，which were compared with another 20 cases of system lupus erythematosus with normal platelets on the aspects of laboratory findings，clinical symptoms，signs，and the differences of disease activity.Results：On the aspects of clinical symptoms and signs such as fever，rash and arthritis，anti ds-DNA，anti NUC，anti HIS，anti U1RNP，anti SSA，anti SSB，anti C1q antibodies positive rate and disease activity，the differences between the two groups were not statistically significant （P 0.05），but for those of systemic lupus erythematosus with medium or severe thrombocytopenia，white blood cell count，hemoglobin concentration and complement C3 concentration were lower，and the positive rate of anti phospholipid antibody was higher，the difference being statistically significant （P 0.05）.Conclusion：For patients with systemic lupus erythematosus with moderate or severe thrombocytopenia，white blood cell count，hemoglobin concentration and complement C3 concentration were lower，and the positive rate of anti phospholipid antibody was higher than those with normal platelets. 　　【Keywords】lupus erythematosus，systemic；thrombocytopenia；anti platelet antibodies 　　系统性红斑狼疮（systemic lupus erythematosus，SLE）是一种多系统损害的慢性自身免疫性疾病，其血清具有以抗核抗体为代表的多种自身抗体[1]。SLE累及血液系统引起血小板减少常见，轻度血小板减少（PLT≤100×1
