PURPOSE: This memorandum delineates the procedures for documenting and monitoring the adequacy of graduate medical resident supervision. 2. SCOPE: This memorandum applies to the Dorn Veterans Affairs Medical Center (VAMC). 3. POLICY: It is the policy of this facility that staff practitioners will actively supervise residents and appropriately document this supervision in the medical record. a. Within the scope of the training program, all residents, without exception, will function under the supervision of staff practitioners. Every VA residency program must ensure adequate supervision is provided for residents at all times. A responsible staff practitioner must be immediately available to the resident in person, or at least on the VA campus, and able to be present within a reasonable period of time, if needed. At a minimum, supervision of residents must be documented in accordance with the procedures listed below. b. As a part of the Quality Management Program, the Residency Review Subcouncil will monitor compliance with the Resident Supervision Policy on a quarterly basis. 4. MONITORING PROCEDURES: a. Results of Medical Record Reviews (1) Charts will be reviewed by designated Medical Records Technicians (MRTs), who will randomly select no less than ten charts per month per monitor from each clinical service line, with the exception of Geriatric and Extended Care (GEC), for which 10% of the charts will be selected (when residents are assigned), due to GEC’s intermittent resident involvement.
