我院2013年门诊退药原因分析及干预建议 　　（上海市同济医院 上海 200065） 　　摘 要 目的：总结2013年我院门诊药房退药情况，分析退药原因及存在问题，提出改进建议。方法：收集2013年退药处方，按退药科室，退药原因及退药种类进行统计，与2012年各类数据比较分析，对比其增减幅度。结果：退药种类中，抗微生物药物、循环系统药物、中成药排前3位，较2012年小幅增长；退药原因中患者拒用、不良反应、生产厂家占前3位，其中患者拒用、不良反应大幅增长；退药科室中普内科、呼吸内科、消化科仍占前3位，均有小幅增长。结论：应加强退药管理，规范退药程序，注意医患之间的沟通，及时总结，减少退药。 　　关键词 门诊 退药 分析 　　中图分类号：R952 文献标识码：C 文章编号：1006-1533（2014）17-0067-03 　　Analysis of the reasons for outpatient drug withdrawal in 2013 　　and intervention suggestions 　　WANG Kaiming， HONG Yang， FANG Yu 　　（Shanghai Tongji Hospital， Shanghai 200065， China） 　　ABSTRACT Objective： To Summarize drug withdrawal in 2013 in the outpatient pharmacy of our hospital and analyze its reasons and problems so as to make recommendations for the improvement. Methods： Prescription for drug withdrawal in 2013 was collected and analyzed according to the different departments and reasons for drug withdrawal， and types of drugs， which were compared with the data in 2012 and their increase or decrease were also compared. Results： Antimicrobial agents， circulatory system drugs and traditional Chinese medicine were ranked in the top three in drug withdrawal species， a slight increase compared with 2012. The patient refusal to use， adverse reactions and manufacturers were ranked in the top three in the reason for drug withdrawal， in which the former two increased greatly. The departments of general internal medicine， respiratory medicine and gastroenterology were also ranked in the top three in the departments of drug withdrawal， which had a slight increase. Conclusion： The management of drug withdrawal should be strengthened and the procedures for drug withdrawal should be standardized. Great attention should be paid to communication on drug withdrawal between doctors and patients so as to reduce drug withdrawal. 　　KEY WORDS outpatient； drug withdrawal； analysis 　　药品是一种特殊商品，其质量直接关系到患者的用药安全和健康。医院门诊药房工作是一个重要岗位，它不仅是一个将药品发放给患者的过程，还在很大程度上影响着医疗质量和医院形象。药品的规范和有效使用直接关系到患者的生命健康，但长期以来，存在着患者要求退（换）药的情
