腹腔镜手术与开腹手术的治疗宫外孕的效果比较 许琼瑜 浙江省慈溪市宗汉医院妇产科，315301 泮月琴 浙江省慈溪市人民医院 315301 摘要： 目的 对腹腔镜手术与开腹手术两种方法治疗宫外孕的术式进行比较，并评价两者对治疗宫外孕的治疗效果。方法 对宫外孕患者行腹腔镜（65例）和开腹手术（54例）的临床资料进行回顾性分析，比较两组的手术中失血量、术后镇痛、肛门排气时间、术后并发症、住院时间、术后发热、术后应用抗生素的时间、腹部疤痕、留置导管时间、术后通畅率等临床资料。结果 腹腔镜组和剖腹手术组在术中失血量(37.5±15.6ml vs 21.3±9.5ml)、术后镇痛(82% vs 26%)、肛门排气时间(32.6±7.8h vs 19.4±4.2h、术后并发症(46% vs 17%)、住院时间(7.6±1.9d vs 3.3±1.1d)、术后发热(74% vs 24%)、术后应用抗生素的时间(6.3±2.2h vs 3.6±1.2)、腹部疤痕(无 vs 有)、留置导管时间(29.5±4.6h vs 7.4±2.3h)、术后通畅率等方面有明显差异(P0．05)。结论 腹腔镜手术治疗宫外孕优于传统开腹组。 关键词 腹腔镜术；开腹手术；宫外孕 Comparison of Treatment Effect between Laparoscopic Surgery and Open Surgery for Ectopic Pregnancy Abstract: objective To compare two surgical methods of ectopic pregnancy between laparoscopic and laparotomy and evaluate therapeutic effect. Methods The clinical data of ectopic pregnancy patients separately from laparoscopic (65 cases) and open surgery (54 cases) were retrospectively analyzed,the data such as total blood loss, postoperative demulcent effect, the anus exhausting time, postoperative complications, duration of hospitalization, fever, and duration of antibiotics, abdominal scar, indwelling catheter time were analyzed between these groups. Results there were significant difference on total blood loss(37.5?5.6ml vs 21.3?.5mlme), postoperative demulcent effect(82% vs 26%), the anus exhausting time(32.6?.8h vs 19.4?.2h t), postoperative complications(46% vs 17%), duration of hospitalization(7.6?.9d vs 3.3?.1dom), fever(74% vs 24%), and duration of antibiotics(6.3?.2h vs 3.6?.24%), abdominal scar(with vs without), indwelling catheter time(29.5?.6h vs 7.4?.3hit) between laparotomy and laparoscopic groups. (P 0.05). Conclusion laparoscopic surgical treatment of ectopic pregnancy is superior to the traditional open group.
