大容量肺灌洗对肺尘埃沉着病患者T淋巴细胞亚群的影响及意义 　　[摘要] 目的 探讨大容量肺灌洗术对尘肺患者外周血中CD3+、CD4+、CD8+ T淋巴细胞的影响及意义。 方法 随机收集我院2014年5月～2015年9月住院行大容量肺灌洗治疗肺尘埃沉着病患者50例及同期我院社区门诊体检健康男性30例，分别检测肺尘埃沉着病患者灌洗前及灌洗后1周外周血中CD3+、CD4+、CD8+ T淋巴细胞表达水平及健康体检男性外周血中CD3+、CD4+、CD8+ T淋巴细胞表达水平，并进行比较分析。 结果 与健康对照组比较，肺尘埃沉着病患者外周血CD3+、CD4+、CD8+ T淋巴细胞水平，CD4+/CD8+比值均下降（P0.05）；经大容量肺灌洗术后1周CD3+、CD4+ T淋巴细胞水平，CD4+/CD8+比值升高（P0.05），CD8+ T淋巴细胞水平降低（P0.05）。 结论 肺尘埃沉着病患者细胞免疫功能下降，大容量肺灌洗术可使肺尘埃沉着病患者CD3+、CD4+ T淋巴细胞水平，CD4+/CD8+比值升高及CD8+ T淋巴细胞水平降低，从而提高肺尘埃沉着病患者细胞免疫功能。 　　[关键词] 肺尘埃沉着病；肺灌洗；T淋巴细胞亚群 　　[中图分类号] R135.2 [文献标识码] A [文章编号] 1674-4721（2016）01（c）-0022-03 　　[Abstract] Objective To explore the effects and significance of large volume lung lavage on CD3+，CD4+ and CD8+ T lymphocytes in the peripheral blood in patients with pneumoconiosis. Methods 50 patients with pneumoconiosis who were hospitalized in our hospital from May 2014 to September 2015 and given large volume lung lavage were randomly collected.30 healthy males undergone physical examination in the community clinic in our hospital at the same period of time were also collected.The expression level of CD3+，CD4+ and CD8+ T lymphocytes in the peripheral blood in patients with pneumoconiosis before lung lavage and 1 week after lung lavage was tested.The expression level of CD3+，CD4+ and CD8+ T lymphocytes in the peripheral blood in healthy males was also tested.Their expression levels were compared and the comparison results were analyzed. Results Compared with the healthy control group，CD3+，CD4+，CD8+ T lymphocytes，and CD4+/CD8+ ratio in the peripheral blood in patients with pneumoconiosis was reduced respectively（P0.05）；After 1 week of large volume lung lavage，CD3+，CD4+ T lymphocytes，and CD4+/CD8+ ratio was increased respectively（P0.05），and CD8+T was reduced（P0.05）.
