Data show that patients present later and with poorer performance status in the UK than in other countries,4,5 and caution needs to be exercised when comparing the cancer survival statistics of the UK with the rest of Europe and certainly the USA.6 Furthermore, the UK has a thorough cancer registry that covers almost 100% of the population, unlike much of the rest of Europe, where population percentages as low as 1% in Germany and 9% in Poland were included in the data.1 The USA also includes only 26·2% of the population in their studies.7 An area bias in many countries also exists—eg, the wealthy north of Italy was predominantly represented in the Italian statistics included in the EUROCARE study.1 Diversity between countries is also present in the recording of diseases, especially for lung cancer, where, in the USA, only histologically con?rmed lung cancer is recorded, thus excluding those without proven histology. Patients without histologically proven disease have a uniformly poor prognosis and their exclusion from the data produces a 25% improvement in 5-year survival.8 In addition to the UK, cancer registries in some Nordic countries also cover almost 100% of the population,1 but a slightly improved survival does seem to exist in these countries compared with the UK. However, no comparison of race or histological subtypes have been made, and further variables, such as in data collection, disease de?nition, and disease follow-up, might also be present, which will have an e?ect on these ?ndings.5 If we stringently analyse and compare the results of the NHS cancer plan with other countries at this early stage, clear problems are likely ro arise. First, patients might seek treatment elsewhere, at much cost and with dubious bene?t. As a result, patient’s anxieties as to whether they are receiving the correct and e?ective treatment from the NHS might be heightened. Additionally, NHS sta?
