266 · 临床研究 · 重庆医学 2013年 1月第42卷第 3期 降钙素原评估呼吸机相关肺炎患者预后的作用 廖雪莲，康 焰△，邓一芸，金晓东，张中伟，周 琰 (四川大学华西医院重症医学科，成都 610041) 摘 要：目的 评估降钙素原(PCT)的动态监测在预测呼吸机相关肺炎(VAP)患者预后中的作用。方法 前瞻性队列研究， 以连续收治的确诊为 VAP的患者为研究对象，随访至28 d。动态监测 VAP患者的 PCT值并计算变化值(△PCT)，比较上述指 标在 生存组与死亡组的差异 ，通过 多因素 回归模型寻找预测 VAP死 亡的独 立危 险 因素 。结果 死亡组第 1、4、7天的 PCT值 明 显 高于生存组 (P0．05)，其预测 VAP死亡的曲线下面积(AUC)值分别 0．732，0．741，0．797；而 PCT 的动态变化值 (△PCT)在 两组患者之间差异无统计学意义(PO．05)；在多因素回归模型中，只有第 7天的全身感染性相关器官功能衰竭评分(S0FA)评 分[0R一6．381(95 CI：2．69O～15．141)]和第 4天的 APACHEⅡ评分EOR=1．853(95 CI：(1．210～1．781)]是预测 VAP患者 死亡 的独 立危 险因素 。结论 PCT 的动 态监测不 能作为评估 VAP患者 28 d死 亡的独立危 险因素。对 VAP患者预后的评估 需 要 临床 的综合 指标 来判 断。 关键词 ：降钙素原 ；呼吸机相 关肺 炎；C反应蛋 白质 doi：10．3969／j．issn．1671-8348．2O13．03．010 文献标识码 ：A 文章编号 ：1671—8348(2013)03—0266—03 Prognostic value of procalcitonin in patients with ventilator-associated pneumonia Liao Xuelian，Kang Yan ，Deng Yiyun，Jin Xiaodong，Zhang Zhongwei，Zhou Yan (Department of Critical Care Medicine，West China Hospital of Sichuan University，Chengdu 610041，China) Abstract：Objective To assess the value of procalcitonin(PCT)in predicting the outcome of venti1ator—associated pneumonia (VAP)patient．Methods Prospective cohort study．Patients consecutively admitted and diagnosed with VAP were studied and fol— lowed unti1 day 28．Dynamic serum PCT 1evels were measured and their alteration between different days(△ PCT)were calculated in death and survival groups．These parameters had been investigated as independent variables in relation to 28一day death as depend— ent variable．Results The increase of PCT levels on day 1，4 and 7 were found in the death group(P％ 0．05)and significantly predic tive of death with area under curve(AUC)of 0．732，0．741 and 0．797，respectively．However，kinetics of PCT levels(△ PCT)did not show a significant difference between the death and survival(P~ 0．05)．Multivariate analysis revealed that only SOFA(day 7) EOR一6．381，(95 CI：2．690～15．141)]and APACHE1I(day 4)EOR一1．468(95％CI：1．210—1．781)]predicted death．Conclu- sion Neither PCT indiv
