Surgical Oncology 1996; 5: 195-200 REVIEW Somatostatin analogue in the localization and treatment of bronchial carcinoid tumours B. K. TEMECK,* S.-S. KOONG,t J. C. REYNOLDSt AND H. I. PASS* Thoreclc Oncology Section, Surgery Branch, National Cancer Institute and tDepartment of Nuclear Medicine, Clinical Center, National Institutes of Health Octreotide scanning is increasingly being used to detect tumours with somatostatin receptors. Moreover, there is growing interest in the use of somatostatin analogues for the treatment of tumours with somatostatin receptors. This review documents the use at our institution of the octreotide scan in three patients with intrathoracic pathology, and comments on overall experience in the literature with this technology. Surgical Oncology 1996; 5: 195-200. Keywords: somatostatin, carcinoid, ectopic ACTH, receptor, analogues. INTRODUCTION Bronchial carcinoid tumours associated with the ectopic ACTH syndrome are typically small, ranging from 0.3 to 1.3 cm in diameter, and may be difficult to detect radiographically [1, 2). Many normal and neoplastic neuroendocrine tissues have somato- stat in binding sites [3, 4), and somatostatin recep- tors have been found in 82% of carcinoid tumours (5). The octreotide scan uses radiolabelled somato- statin analogues C 23 1-Tyr-3-octreotide and 1111n_ pentatreotide) for the in vivo localization of tumours with somatostatin receptors [6, 7). This report presents three patients with bronchial carcinoid tumours in whom the octreotide scan helped in preoperative evaluation. CASE REPORT 1 EP was a 45-year-old white female who began, in 1990, to have several spontaneous fractures, weight gain, easy bruisability, fatigue, menstrual irregularity, emotional lability, depression, skin depigmentation, hypertension, glucose intolerance Correspondence: Harvey I.
