供应商保证书 关于有害物质减免符合性 Supplier Guarantee Letter regarding Hazardous Substances Compliance 供应商名称（保证者） Name of Supplier (Guarantor)供应商营业执照注册号 Registration No. of Supplier供应商地址 Registered Address of Supplier制造商名称 Name of Manufacturer制造商地址 Registered Address of Manufacturer 供应商承诺向快捷达及其关联公司（以下统称为“快捷达”，名单见附录A）所销售的所有产品、部品、物料及附属部品，符合快捷达所在地有关有害物质管理法律法规的要求及国际有关有害物质管理法律法规的要求，特此保证如下： Supplier undertakes to commit that all products, parts, raw materials and accessories delivered to Aztech and its affiliated companies as mentioned in Annex A (collectively called as “Aztech”, ) shall meet Aztech’s requirement of relevant hazardous substances prohibited by local and international laws/regulations. Supplier hereby, guarantees the following: 符合快捷达所在地及国家所适用如《电子信息产品污染控制管理办法》(包括现行以及后续不时修订的内容)等有关法律法规的要求。 Comply with all applicable laws/regulations of the country in which Aztech’s Manufacturing Plant is located, such as《Measures for Administration of the Pollution Control of Electronic Information Product from China》and all its subsequent revisions. 符合国际所适用如EU Directive 2011/65/EU、EU Directive 2012/19/EU、EU Regulation(EC) No 1907/2006、Packaging Directive 94/62/EC、EU Directive 2006/66/EC 及它们后续修订的内容等有关法律法规的要求（有害物质管控清单及限制值见附录B）。 Comply with all applicable international laws/regulations, such as EU Directives 2011/65/EC, EU Directive 2012/19/EU, EU Regulation (EC) No 1907/2006, Packaging Directive 94/62/EC, EU Directive 2006/66/EC and all its subsequent revisions (Regulated Hazardous Substances List and Limit Value as mentioned in Annex B). 供应商已充分理解并承诺符合快捷达的有害物质减免管理标准（文件编号：OPI-024），此有害物质减免管理标准将依据国内国际法律法规的最新要求及快捷达客户的最新要求进行不时的后续修订。 Supplier fully understands and is committed to comply with Aztech’s HSF Management Standard (Document No.: OPI-024).
