Abstract
Background:Studieshavesuggestedthatnumberofsiblingsandbirthorderisassociatedwithobesity.However,studies
combiningtheseexposuresareneeded.Thisstudyaimedatinvestigatingobesityinchildrenandyoungadultsinregardto
differentcombinationsoffamilysizeandbirthorder.
Methods:Twocohortsselectedfromthegeneralpopulationwereinvestigated:TheCopenhagenSchoolHealthRecords
Register(CSHRR)andaDraftBoard(DB)samplewithmeasuredheightsandweightsinchildhood(age13years)andyoung
adulthood(age19years),respectively.Informationonbirthorder,numberofsiblings,andrelevantcovariateswereavailable
on29327children,aswellason323obeseyoungmenand575randomlyselectedcontrolsofyoungmenrepresenting
approximately58000.Therelationbetweennumberofsiblingsandbirthorder,respectively,andhavingaBodyMassIndex
(BMI)z-scoreaboveorequaltothe95thpercentileinchildhoodorhavingaBMIofatleast31.00kg/m2inyoungadulthood
wasanalysedusinglogisticregressionanalysesadjustedforrelevantconfounders.
Results:Onlychildrenhadsignificantlyhigheroddsofobesitybothinchildhoodandinyoungadulthoodcomparedwith
children with siblings, odds ratio (OR)=1.44 (95% Confidence Interval (CI): 1.26–1.66) and OR=1.76 (95% CI: 1.18–2.61),
respectively.Noassociationbetweenfirst-bornstatusandobesitywasfound.TheORoflast-bornchildrenbeingobesewas
alsosignificantlyincreasedinchildhood,e.g.OR=1.93(95%CI:1.0923.43)ofobesityiflast-borninafamilyoffourchildren.
This was not found in young adulthood. Additionally, higher spacing to previous sibling (average 1872 vs. 1303 days;
p=0.026infourchildrenfamilies)wasobservedinobeselast-borncomparedtonon-obeselast-
