Methodology/Principal Findings: Commencing September2004,weconductedanecologicalstudythatincluded active surveillanceforskininfectionsamongstchildrenaged,15yearsinfiveremoteEastArnhemcommunitiesoverathreeyear period. Screening was undertaken by trained local community workers, usually accompanied by another project team member,usingastandarddatacollectionform.Skininfectionswerediagnosedclinicallywiththeaidofapictorialflipchart developed for the purpose. Topical 5% permethrin was provided for age-eligible children and all household contacts wheneverscabieswasdiagnosed,whilstthosewithpyodermawerereferredtotheclinicfortreatmentinaccordancewith currentguidelines.Inaddition,annualmassscabiestreatment(5%permethrincream)wasofferedtoallcommunityresidents in accordance with current guidelines but was not directly observed. Pyoderma and scabies prevalence per month was determinedfrom6038skinassessmentsconductedon2329children.Pyodermaprevalencedroppedfrom46.7%atbaseline toamedianof32.4%(IQR28.9%–41.0%)duringthefollow-upperiod–anabsolutereductionof14.
