AssociationbetweenInsomniaSymptomsand HemoglobinA1c LevelinJapaneseMen YukoKachi*,MutsuhiroNakao,TakeakiTakeuchi,EijiYano DepartmentofHygieneandPublicHealth,SchoolofMedicine,TeikyoUniversity,Tokyo,Japan Abstract Background:TheevidenceforanassociationbetweeninsomniasymptomsandbloodhemoglobinA1c (HbA1c)levelhas beenlimitedandinconclusive.Theaimofthisstudywastoassesswhethereachsymptomofinitial,middle,andterminal insomniainfluencesHbA1c levelinJapanesemen. Methods: This cross-sectional study examined 1,022 male workers aged 22–69 years with no history of diabetes at a Japanesecompany’sannualhealthcheck-upinApril2010.HighHbA1cwasdefinedasabloodlevelofHbA1c$6.0%.Three typesofinsomniasymptoms(i.e.,difficultyininitiatingsleep,difficultyinmaintainingsleep,andearlymorningawakening) fromthepreviousmonthwereassessedby3responses(i.e.,lastingmorethan2weeks,sometimes,andseldomornever [referencegroup]). Results: The overall prevalence of high HbA1c was 5.2%. High HbA1c was positively and linearly associated with both difficultyinmaintainingsleep(Pfortrend =.002)andearlymorningawakening(Pfortrend =.007).Morespecifically,after adjusting for potential confounding factors, high HbA1c was significantly associated with difficulty in maintaining sleep lastingmorethan2weeks(adjustedoddsratio,6.79[95%confidenceinterval,1.86–24.85])orsometimes(2.33[1.19–4.55]). HighHbA1cwasalsosignificantlyassociatedwithearlymorningawakeninglastingmorethan2weeks(3.96[1.24–12.59]). Conclusion:Insomniasymptoms,particularlydifficultyinmaintainingsleepandearlymorningawakening,werefoundto haveacloseassociationwithhighHbA1c inadose-responserelationship.
