Abstract
Background:Thisreportpresentsasystematicreviewofscientificliteraturepublishedbetween1990–2010relatingtothe
frequencyofhumanbrucellosis,commissionedbyWHO.Theobjectivesweretoidentifyhighqualitydiseaseincidencedata
tocomplementexistingknowledgeoftheglobaldiseaseburdenand,ultimately,tocontributetowardsthecalculationofa
Disability-AdjustedLifeYears(DALY)estimateforbrucellosis.
Methods/PrincipalFindings:Thirtythreedatabasesweresearched,identifying2,385articlesrelatingtohumanbrucellosis.
Basedonstrictscreeningcriteria,60studieswereselectedforqualityassessment,ofwhichonly29wereofsufficientquality
fordataanalysis. Datawereonlyavailablefrom 15countriesintheregions ofNorthernAfrica andMiddleEast, Western
Europe,CentralandSouthAmerica,Sub-SaharanAfrica,andCentralAsia.Halfofthestudiespresentedincidencedata,sixof
whichwerelongitudinalprospectivestudies,andhalfpresentedseroprevalencedatawhichwereconvertedtoincidence
rates. Brucellosis incidence varied widely between, and within, countries. Although study biases cannot be ruled out,
demographic,occupational,andsocioeconomicfactorslikelyplayarole.Aggregateddataatnationalorregionallevelsdo
notcapturethesecomplexitiesofdiseasedynamicsand,consequently,at-riskpopulationsorareasmaybeoverlooked.In
manybrucellosis-endemiccountries,healthsystemsareweakandpassively-acquiredofficialdataunderestimatethetrue
diseaseburden.
Conclusions:Highqualityresearchisessentialforanaccurateassessmentofdiseaseburden,particularlyinEasternEurope,
the Asia-Pacific, Central and South America and Africa where data are lacking. Providing formal epidemiological and
statistical training to researchers is essential for improving study quality. An integrated approach to dis
