KarahocaandMomparlerClinicalEpigenetics2013,5:3 /content/5/1/3 REVIEW OpenAccess Pharmacokineticandpharmacodynamicanalysis of5-aza-2’-deoxycytidine(decitabine)inthe designofitsdose-scheduleforcancertherapy MetinKarahoca1andRichardLMomparler1,2* Abstract 5-Aza-2′-deoxycytidine(5-AZA-CdR,decitabine),anepigeneticdrugthatinhibitsDNAmethylation,iscurrentlyused totreatmyelodysplasticsyndrome(MDS),andisunderinvestigationfortreatingacutemyeloidleukemia(AML)and othermalignancies.5-AZA-CdRcanreactivatetumorsuppressorgenessilencedbyaberrantDNAmethylation,a frequenteventinalltypesofcancer.Becausethisepigeneticchangeisreversible,itisagoodtargetfor5-AZA-CdR therapy.Wehavereviewedthepreclinicaldataof5-AZA-CdRtoanalyzetheconcentrationsandexposuretimes requiredtoeradicatecancerstemcells.Weanalyzedthedose-schedulesusedinanimalmodelsthatshowpotent antineoplasticactivityof5-AZA-CdR.Weattemptedtocorrelatethepreclinicaldatawiththeresponsesobtainedin clinicaltrialsof5-AZA-CdRinpatientswithcancer.Thepharmacokineticsanddrugdistributionof5-AZA-CdRare keyparametersbecauseadequatetherapeuticdruglevelsarerequiredtoeliminatecancerstemcellsinall anatomiccompartments.Theplasmahalf-lifeof5-AZA-CdRinhumansisapproximately20minutesduetothehigh levelsintheliverofcytidinedeaminase,theenzymethatinactivatesthisanalogue.Thisprovidesarationaletouse aninhibitorofcytidinedeaminaseincombinationwith5-AZA-CdR.Low-dose5-AZA-CdRiseffectiveforMDSand AMLandcaninducecompleteremissions(CR).However,maintenanceofCRwithlow-dose5-AZA-CdRisdifficult. Basedonanalysesofpreclinicalandclinicaldata,lowdose5-AZA-CdRhasthepotentialtobeaneffectiveformof therapyinsomepatientswithcancer.Forpatientswhodonotrespondtolowdosetherapywerecommend dose-intensivetreatmentwith5-AZA-CdR.Patientswhoarecandidatesforintensivedose5-AZA-CdRshouldhavea goodbonemarrowstatussoastopermitadequaterecoveryfrommyelosuppression,themajortoxicityof5-AZA-CdR.
