Mostofthesedatawereobtainedfromadultstudies.Theaimofourstudywastoscreenforliverdiseaseamong type1diabeticchildren. Methods:Childrenwithtype1diabetesfollowinginclinichavebeenexaminedforexistenceofliverdisease,from November2008toNovember2009.Allweresubjectedtothefollowing:History,physicalexamination,liverfunction tests,fastinglipidprofile,HbA1C,andultrasoundoftheliver.Ahyperechogenicliverand/orhepatomegalyon ultrasoundwereattributedmostlikelytoexcessglycogenorfatintheliver,afternegativeextensivework-upto ruleoutotherunderlyingliverdisease. Results:106childrenwithtype1diabeteswerestudied:agerangedbetween8monthsto15.5years,sixtytwo patientswerefemales.Twentytwopatients(21%)wereidentifiedtohaveabnormalfindingsonultrasoundofthe liver:10patientshadhepatomegalyand12hadhyperechogenicliver.Thegroupwithhyperechogenicliverhad poorerglycemiccontrolthanpatientswithnormalliver(MeanHbA1c12.14%Vs10.7%;Pvalue=0.09). Hyperechogenicliverresolvedin60%at6monthsfollow-upuponachievingbetterglycemiccontrol. Conclusions:Hyperechogenicliverand/orhepatomegalyarenotuncommoninchildrenwithtype1diabetesand tendtobemoreprevalentamongchildrenwithpoorglycemiccontrol.Type1diabetesrelatedhepatopathyis reversiblebyoptimizingglycemiccontrol.Becauseofitssafety,andreliability,ultrasoundcanbeusedtoscreenfor hepatopathyintype1diabeticchild.
