Aggarwaletal.HarmReductionJournal2012,9:4 /content/9/1/4(18January2012) Aggarwaletal.HarmReductionJournal2012,9:4 /content/9/1/4 REVIEW OpenAccess Psychoactivesubstancesandthepoliticalecology ofmentaldistress SunilKAggarwal1*,GregoryTCarter2,CraigZumbrunnen3,RichardMorrill3,MarkSullivan4,5 and JonathanDMayer6,7,8,9,10 Abstract Thegoalofthispaperistobothunderstandanddepathologizeclinicallysignificantmentaldistressrelatedto criminalizedcontactwithpsychoactivebioticsubstancesbyemployingaframeworkknownascriticalpolitical ecologyofhealthanddiseasefromthesubdisciplineofmedicalgeography.Thepoliticalecologyofdisease frameworkjoinsdiseaseecologywiththepower-calculusofpoliticaleconomyandcallsforsituatinghealth-related phenomenaintheirbroadsocialandeconomiccontext,demonstratinghowlarge-scaleglobalprocessesareat workatthelocallevel,andgivingdueattentiontohistoricalanalysisinunderstandingtherelevanthuman- environmentrelations.Criticalapproachestothepoliticalecologyofhealthanddiseasehavethepotentialto incorporateever-broadeningsocial,political,economic,andculturalfactorstochallengetraditionalcauses, definitions,andsociomedicalunderstandingsofdisease.Inspiredbythepatient-centeredmedicaldiagnosis critiquesinmedicalgeography,thispaperwilluseacriticalpoliticalecologyofdiseaseapproachtochallenge certainprevailingsociomedicalinterpretationsofdisease,ormorespecifically,mentaldisorder,foundinthefieldof substanceabusediagnosticsandtherelatedAmericanpunitivepublicpolicyregimesofsubstanceabuse preventionandcontrol,withregardstotheuseofbioticsubstances.Itwilldothisbyfirstcriticallyinterrogating theconceptof“substances”andgroundingtheminanecologicalcontext,reviewingthehistoryofboththe developmentofmodernsubstancecontrollawsandmodernsubstanceabusediagnostics,andunderstandingthe biogeographicdimensionsofsuchapproaches.Itcloseswithproposinganon-criminalizingpublichealthapproach forregulatinghumanclosecontactwi
