Wangetal.AIDSResearchandTherapy2011,8:12 /content/8/1/12 RESEARCH OpenAccess Rectalmicrobicides:clinicallyrelevantapproach tothedesignofrectalspecificplacebo formulations LinWang1,RogerLSchnaare1,CharleneDezzutti1,2,PeterAAnton3,LisaCRohan1,2,4* Abstract Background:Theobjectiveofthisstudyistoidentifythecriticalformulationparameterscontrollingdistribution andfunctionfortherectaladministrationofmicrobicidesinhumans.Fourplaceboformulationsweredesigned withawiderangeofhydrophiliccharacteristics(aqueoustolipid)andrheologicalproperties(Newtonian,shear thinning,thermalsensitiveandthixotropic).Aqueousformulationsusingtypicalpolymerstocontrolviscositywere iso-osmoticandbufferedtopH7.Lipidformulationsweredevelopedfromlipidsolvent/lipidgellingagentbinary mixtures.Testingincludedpharmaceuticalfunctionandstabilityaswellasinvitroandinvivotoxicity. Results:Theaqueousfluidplacebo,basedonpoloxamer,wasfluidatroomtemperature,thickenedandbecame shearthinningat37°C.Theaqueousgelplacebousedcarbopolasthegellingagent,wasshearthinningatroom temperatureandshowedatypicaldecreaseinviscositywithanincreaseintemperature.Thelipidfluidplacebo, myristylmyristateinisopropylmyristate,wasrelativelythinandtemperatureindependent.Thelipidgelplacebo, glycerylstearateandPEG-75stearateincaprylic/caprictriglycerides,wasalsoshearthinningatbothroom temperatureand37°Cbutwithsignificanttimedependencyorthixotropy.Allformulationsshowednorectal irritationinrabbitsandwerenon-toxicusinganexvivorectalexplantmodel. Conclusions:Fourplaceboformulationsrangingfromfluidtogelinaqueousandlipidformatswitharangeof rheologicalpropertiesweredeveloped,tested,scaled-up,manufacturedundercGMPconditionsandenrolledina formalstabilityprogram.Clinicaltestingoftheseformulationsasplaceboswillserveasthebasisforfurther microbicideformulationdevelopmentwithdrug-containingproducts.
