中华整形外科杂志 2015 年 5 月第 31 卷第 3 期 Chin J Plast Su眩， May 2015 , Vol. 31 , No. 3 161 ·临床论著· 下唇唇红复合组织瓣转移修复 1/4 上唇 唇红缺损 赵竞伊靳小雷滕利徐家杰卢建建张超 [摘要] 目的 探讨一种蒂位于口角的下唇唇红复合组织瓣转移修复 1/4 上唇唇红缺损的再 造方法，以避免传统交叉唇瓣转移后造成的进食、言语、口腔卫生等困难。方法 手术分两期进行。 一期手术将蒂部位于患侧口角的下唇动脉血管蒂携带的下唇唇红蒙古膜瓣转移至上唇半侧缺损处， 远端部分唇红瓣去表皮后再造唇珠，供区直接关闭;二期手术将唇红瓣断蒂并进行局部修整。结果 共治疗 6 例患者，一期手术后转移交叉唇红瓣血运良好;二期断蒂后唇红瓣完全存活。再造上唇 丰满，基本对称，外形自然，口角形态未受破坏，供区癫痕不明显。结论 改良的交叉唇红瓣克服了 传统蒂位于中央的交叉唇红瓣转移后给患者带来的诸多不便和痛苦，手术方法简单，血运可靠，术 后外形和功能良好。 [关键词} 交叉唇瓣; 上唇缺损 Reconstruction of the 114 defect on upper- Iip vermilion with a lower- Iip vermilion compound tissue flap Zhαo lingyi , lin Xiaolei. China Corresponding αuthor: lin Xiaolei , Emαil :jinyuyizxxy@ 126. com [Abstract] Objective To investigate the reconstruction of 1/4 defect on upper-lip vermilion with a lower-lip vermilion compound tissue flap pedicled at oral commissure. Methords At the first stage. the lower lip mucosal flap pedi.cled by inferior labial arteηwas transposed to reconstruct the defect on upper lip vermilion and tubercle. The defect at the donor site was closed directly. At the second stage. the flap pedicle was cut off and revised. Results 6 patients were treated with satisfactory aesthetic results. All the flaps survived completely. The oral commissure kept normal with no obvious scar at the donor sites. Conclusions The modified crosslip vermilion flap pedicled at oral commissure has the advantages of avoiding inconvenience in feeding , speaking and cleaning.
