RoseNathan*andMathewAlexanderMwanyangala Abstract Background:Theconceptofcontinuumofcarehasrecentlybeenhighlightedasacoreprincipleofmaternal, newbornandchildhealthinitiatives,andasameanstosavelives.However,evidencehasconsistentlyrevealed thataccesstocareduringandpostdelivery(intraandpostpartum)remainsachallengeinthecontinuumofcare framework.Inplaceswhereskilleddeliveryassistanceisexclusivelyavailableinhealthfacilities,accesstohealth facilitiesiscriticaltothesurvivalofthemotherandhernewborn.However,littleisknownabouttheassociationof placeofdeliveryandsurvivalofneonates.ThispaperuseslongitudinaldatageneratedinaHealthand DemographicSurveillanceSysteminruralSouthernTanzaniatoassessassociationsofneonatalmortalityandplace ofdelivery. Methods:Threecohortsofsingletonbirths(born2005,2006and2007)wereeachfollowedupfrombirthto28 days.Placeofbirthwasclassifiedaseither“healthfacility”or“community”.Neonatalmortalityrateswereproduced foreachyearandbyplaceofbirth.Poissonregressionwasusedtoestimatecruderelativerisksofneonataldeath byplaceofbirth.Adjustedratioswerederivedbycontrollingformaternalage,birthorder,maternalschooling,sex ofthechildandwealthstatusofthematernalhousehold. Results:Neonatalmortalityforhealthfacilitysingletondeliveriesin2005was32.3per1000livebirthswhilefor thoseborninthecommunityitwas29.7per1000livebirths.In2006,neonatalmortalityrateswere28.9and26.9 per1,000livebirthsfordeliveriesinhealthfacilitiesandinthecommunityrespectively.In2007neonatalmortality rateswere33.2and27.0per1,000livebirthsforthoseborninhealthfacilitiesandinthecommunityrespectively. Neonatesborninahealthfacilityhadsimilarchancesofdyingasthoseborninthecommunityinallthethree yearsofstudy.Adjustedrelativerisks(ARR)forneonataldeathborninahealthfacilityin2005,2006and2007were 0.99(95%CI:0.58-1.70),0.98(95%CI:0.62-1.5
