手指延长器在治疗桡骨远端不稳定骨折中的应用 作者：刘成　肖军　杨超　李坚　付纳新　孟桢桢　彭风平 【关键词】 ,手指延长器 【摘要】 目的 评价手指延长器在 治疗 桡骨远端不稳定骨折撑开复位固定的临床效果及 应用 价值。 方法 采用手指延长器的可调控逐步撑开复位固定方法，部分结合克氏针撬拨复位治疗桡骨远端不稳定骨折 102例，按 AO/ ASIF分型： A3型 61例， C2型 27例， C3型 14例。治疗方法中单纯撑开复位固定 76例，加用经皮克氏针撬拨复位 26例。结果 102例行此法治疗中， 3例改行切开复位植骨加钢板内固定。余 99例中，解剖复位 57例，功能复位 32例，接近功能复位 10例；关节功能恢复优 73例,良 18例，可 8例，无严重并发症。结论 手指延长器的可调控撑开复位固定治疗桡骨远端不稳定骨折是一种操作简单、固定确实、疗效满意、并发症少的方法。 　　【关键词】 手指延长器；撑开复位固定；桡骨远端骨折 　　Application of finger extender in treating unstable fracture of distal radius 　　　【Abstract】 Objective To appraise the clinical effect and applied value of the finger extender in treating unstable fracture of the distal radius which was operated unfold reduction and fixation. Methods The adoption of finger lengthening device can regulate,control and restore to the throne the regular method progressively,combine Kuntscher Nail partly which treat unstable fracture of the distal radius carry 102 cases.According to AO/ASIF dividing type:A3 type 61,C2 type 27,C3 type 14.Of treatment methods,simply unfold reduction and fixation were 76,using Kuntscher Nail to unfold reduction through skin were 26.Results 102 routine adopt this treat,3 were planted bone add plate regular to internal fixation. Off surplus 99 cases,57 cases were anatomical reduction,32 cases were functioncal reduction,11 cases close to functioncal reduction. The joint functionality is restored to the throne,73 cases were excellent,18 cases were fine,8 cases were badworse,no serious complication.Conclusion The finger extender can regulate,control and restore to the throne the regular to treat unstable fracture of the distal radius. The operation to be simple,stable fixation. The curative is satified,complication few methods. 　　【Key words】 finger extender；unfold reduation and fixation；fracture of the distal radius 　　不稳定型桡骨远端骨折是一种临床常见的骨折，因大多涉及桡腕关节或下尺桡关节面，易导致畸形愈合和腕关节功能减退。传统的手法复位石膏外固定难以达到令人满意的疗效，切开复位或外固定支架固定又显创伤大，操作复杂。我科 1998年 3月～2004年 12月将手指延长器用于 102例严重粉碎性桡骨远端骨折的治疗，取得了满意的临床疗效。 　　1 资料与方法 　　1
