经乳晕切口乳房松垂整形术的临床应用研究 作者：陈忠存 李庆高尚 王永洁 【摘要】 目的 探求对乳房松垂的整复 治疗 行之有效的手术方式。 方法 对正常体积的乳房松垂，选择新月形或双环形切口， 将乳腺上缘固定于相应的胸壁上。对乳房松垂伴萎缩的， 应用 新月形切口，悬吊乳腺的同时行隆乳术。对乳房松垂伴巨乳的，采用双环切口行巨乳缩小乳腺悬吊术。结果 38例受术者随访3～6个月，乳房挺拔，丰满，切口瘢痕不明显，未出现乳头乳晕坏死及乳晕增大，外缘放射状皮肤皱折于术后3～6个月均消失，医患双方均感满意。结论 经乳晕切口乳房松垂整形术术式设计简单，血供及乳腺悬吊固定良好。切口隐蔽，术后瘢痕不明显。乳头乳晕感觉正常，乳房塑性好，是治疗乳房松垂的一种良好方法。 【关键词】 乳房松垂 乳房松垂整形术 Clinical application research of mastopexy through the areola of breast 【Abstract】 Objective To seek a kind of effective surgical operation method to treat women with the breast descensus.Methods To the normal volume breast descensus，select crescent-shaped or two annular incisions，and the breast superior border was fixed on the standard place.To the breast descensus company with atrophy，using crescent-shaped incisions，sling the breast，and at the same time with the augmentation mammoplasty.To the ptotic breast company with macromastia，using the two annular incisions with reduction mammoplasty.Results The 38 recEived operation were followed up for 3～6 months，and the breasts were tall，straight and plentiful，the scars of incisions were not apparent，the nipple and the areola of breast did not appear necrosis，and the areola of breast was not enlarged，the outer radiation skin wrinkles all disappear in 3～6 months after operation，the surgeons and the patients all feel satisfactory.Conclusion The suspension mammoplasty through the areola of breast design is simple，the blood supply and breast suspension fixation are good，the incisions are secluding and the scar is not apparent.The sensation of the nipple and areola of breast is normal，and the mammoplasty is good，and it is a kind of good method to treat breast descensus. 【Key words】 ptotic breast mastopexy 乳房松垂整形术自Thorek于1922年实施以来已有近百年的 历史 ，其方法 文献 报道的有数十种之多。常用的方法如Mckissocks法、Skoogs法，国内的有宋氏法、上蒂瓣法、下蒂瓣法等。但多数的方法，由于其术后瘢痕明显，而往往使患者望而却步。近年来，经乳晕乳房松垂整形术的应用取得了较为满意的效果。在此方面，笔者也进行了尝试，现报告如下。 1 资料与方法 1.1 一般资料 本组共38例，76只乳房，均为健康女性，年龄29～44岁，平均35岁。其中正常体积的乳房下垂17例，34只乳房，均行乳腺悬吊固定术；乳房下垂伴萎缩的1
