INDIVIDUAL TAX RESIDENCY SELF CERTIFICATION FORM 個人稅務居民身份自行認證 Please read these instructions before completing the form. 填寫自行證明書前，請先參閱下列指引。 Important Notes 注意事項 1. This is a self certification form provided by you to DBS Vickers (Hong Kong) Ltd (“DBSV”) for the purposes of (i) automatic exchange of financial account information; and (ii) US federal income tax. 這是由您向星展唯高達香港有限公司( 「星展唯高達」)提供的自行認證，以作(i) 自動交換財務帳戶資料及(ii) 美國聯邦所得稅之用途。 2. The data collected may be transmitted by DBSV (i) for the purpose of paragraph 1(i) above, to the Inland Revenue Department for transfer to the tax authority of another jurisdiction;and (ii) for the purpose of paragraph 1(ii) above, to the US Internal Revenue Service in the case of US persons. 星展唯高達可把收集所得的資料交給稅務局，稅務局會將資料轉交到另一稅務管轄區的稅務當局；或倘若您 是美國人，資料將交給美國稅務局。 3.
