中国药物应用与监测 2014 年12月第11卷第6 期 Chinese Journal of Drug Application and Monitoring, Vol.11, No.6, December 2014 ·药学服务 · 品管圈在降低我院门诊药房药品过期损耗中的应用 崔晋涛，刘　萍，曹媛媛，柴　栋（解放军总医院海南分院药剂科，海南 三亚 572013 ） [摘要]　目的：探讨品管圈活动对降低药品过期造成的药品损耗的效果。方法：成立品管圈，按照品管圈的步骤实施各项活 动，运用头脑风暴法分析产生药品过期损耗高的原因，采取鱼骨图及关联图手法探讨解决的对策并实施，评价有形成果（过 期药品报损率），采用雷达图评价无形成果（圈能力）。结果：我院过期药品损耗率高的主要原因是医生获得药品信息不畅， 药师工作有疏漏，频繁更换药品规格、厂家、批号及商业配送近效期药品等。针对上述原因制定了相应的工作制度及多项措 施，并纳入药品养护的工作流程。定期组织对医生、药师进行业务知识、相关制度强化培训等有效措施。活动后成效显著， 过期药品报损率由 0.230% 降低至0.021%。药师在品管圈手法、责任感、团队精神、沟通及协调能力等方面都有大幅度提高。 结论：品管圈活动对我院降低药品过期报损率发挥了重要作用。 [关键词]　品管圈；门诊药房；过期药品；报损率 [ 中图分类号]　R95　　 [ 文献标识码]　A　 [ 文章编号]　1672 – 8157(2014)06 – 0383 – 04 Application of quality control circle in reducing the loss rate of expired medicine in our hospital CUI Jin-tao, LIU Ping, CAO Yuan-yuan, CHAI Dong(Department of Pharmacy, Hainan Branch of the Peoples Liberation Army General Hospital, Sanya 572013, China) [ABSTRACT]　Objective: To explore the effect of quality control circle (QCC) activity in reducing the medicine loss caused by expiration. Methods: Each step was carried out according to QCC. Specifically, brainstorming method was applied to analyze the reason of a high level for the expired medicine, fishbone diagram and association graph were used to explore solutions and evaluate tangible results (reported loss rate of expired medicine), while radar diagram was adopted for assessing intangible outcomes (ability of QCC) and formulating standardized process. Results: The results strongly suggested that the major reasons for high loss rate of expired medicine were the inefficient communication with doctors about medicine information, the careless omission in pharmacists work, the frequent change of medicine specification, producer, batch number and the commercial distribution of medicines which were close to the expired date, etc. Corresponding working system and many measures were brought into working process o
