METHODS ：Based on the practice of our hospi- tal ，feasible working mode of clinical pharmacists was summarized in the view of cooperation between clinical pharmacists and pharmacists of wards and pharmacy by means of case study. RESULTS CONCLUSIONS ：The personnel posts of clinical pharma- cists should be included in the management of inpatient pharmacy. Clinical pharmacists provide whole course pharmaceutical care for doctors ，nurses and patients by means of participating in clinical practice of responsibility department ，accumulating experience of clinical drug use ，providing pharmaceutical care and returning visit for cases of other departments requiring consultations. Clini- cal practice is extended to teaching and research of clinical pharmacy. Established working mode of clinical pharmacists contributes to safe and effective drug use and the achievement of work value of clinical pharmacists.
